Officer DESCRIPTION: The contractor shall provide all personnel,
equipment, tools, materials, supervision, and other items or services
necessary to perform asbestos abatement and disposal from buildings
6347 and 6348, Lackland AFB TX. Contractor shall remove all asbestos
floor tile including mastic, exterior asbestos siding including tar
paper, asbestos insulation from air conditioning systems. The
contractor is responsible for any permit and/or fee requirements. Note
the following additional requirements: 1. Compliance Requirements The
contractor shall comply with all federal, state, and local regulatory
agency requirements, environmental statutes, policies, and regulations
including, but not limited to those cited below: a) Occupational Safety
and Health Administration (OSHA) Act b) Department of Transportation
(DOT) Regulations. c) National Environmental Policy Act (NEPA) d). U.S.
Environmental Protection Agency (EPA), Title 40, code of Federal
Regulations. e) Title 30, Texas Administrative Code (TAC). f) Lackland
AFB Spill Prevention and Response Plan, dated 1 April 1994. g) The
Texas Natural Resource Conservation Commission (TNRCC) Office of
management memorandum, dated 11 Jan 1996. 2. State Notification The
contractor shall be responsbile for the proper completion of the Texas
Department of Health Demolition/Renovation Notification Form and will
also act as the 37th CES representative and sign the signature block
for the building onwer/operator and shall be responsible for all
aspects of the notificaiton form, including, but not limited to,
content and submission dates. The contractor will provide 37th CES/CEV
with a copy of the notification form for review and approval before
sending it to the Texas Department of Health. 3. Submittals The
following documentation must be submitted with 10 calendar days after
the award date and prior to the commencement of any work; a) Asbestos
Work Plan b) Employee Training Certificates c) Employee physicals All
waste maifest procedures will be accomplished in accordance with local,
state, and federal laws and regulations. Period of performance will be
30 calendar days from the date of award. A SITE VISIT IS SCHEDULED FOR
16 SEP 96 AT 9:00 A.M. CONTRACTORS ARE TO BE AT 37 CONTRACTING SQUADRON
(BLDG 5450) LACKLAND AFB AT SCHEDULED TIME. CONTRACTORS ARE TO RSVP IF
THEY WILL ATTEND TO THE NUMBER LISTED ABOVE Offers will bid the
project as one lumb sum job. This is a COMBINED SYNOPSIS/SOLICITATION
for commerical services prepared in accordance to the format in Subpart
12.6, as supplemented with additional information included in this
notice. This announcement consititutes the ONLY SOLICITION; proposals
are being requested and a written solicition WILL NOT be issued. This
solicition is issued as a request for quotation. This procurement is
issued utilizing the size standard in FAR 19 under Standard Industrial
Code 8744. This solicitation document and incorporated provisions and
clauses are thos in effect through Federal Acquisition Circular 90-32.
The provisions at FAR 52-212-1, Istructions to Offerors-Commercial,
applies to this acquisition. The clause at 52.212-5, Contract Terms and
Conditions Required to Implement Statutes or Executive
Orders-Commercial Items, applies to this solicitation. Specifically,
the following cited clauses are applicable to this solicition: FAR
52.222-26, Equal Opportunity; 52.222-35, Affirmative Action for Speical
Disabled and Vietman Era Veterns; 52.222-36, Affirmative Action for
Handicapped workers; 52.222-37, Employment Reports on Special Disabled
Veterns on the Vietnam Ear; 52.222-41, Service Contract Act of 1965 as
Amended; 52.222-8, Payrolls & Basic Records. The clause at FAR
52.219-6, Notice of Total Small Business Set-Aside applies to this
solicitation. Wage Decision: Pursuant to the The Service Contract Act,
as amended, Wage Determination No: 94-2521, Revision 4, Dated 95 April
21 will apply to the solictation/contract contemplated hereby. Offerors
must include with their offer a complete copy of the provisions af FAR
52.212-3, Offerors Representation and Certification-Commercial Items
(This can be obtained from Contracting Office). The clause at FAR
52.212-4 Contract Terms and Conditions-Commercial Items, applies to
this solicitation. Award will be based on the lowest priced quotation
from a responsible contractor in accordance with FAR 9.104-1. Quotes
may be submitted in writing or faxed to the point of contact indicated
