FAX:305/324-3280 This is a combined synopsis/solicitation for
commercial items prepared in accordance with the format in FAR Subpart
12.6, as supplimented with additional information included in this
notice. This announcement constitutes the only solicitation. Proposals
are being requested and a written solicitation will not be issued.
This solicitation no. RFP 546-28-97 is issued as a Request For
Proposals. Incorporated provisions and clauses are those in effect
throught Federal Acquisition Circular 41. The Standard Industrial Code
(SIC) is 3829 and the small business size standard is 500 employees.
Contract will be firm, fixed-price. Requirement is to provide to the VA
Medical Center, Miami, Florida, FOB destination, a Xenon cerebral blood
flow analysis system for an existing General Electric high speed
advantage CT scanner. System allows CT scanner to do functional and
quantitative blood flow exams via delivery system monitor and
control,CT file transfer via network, CBF analysis, CBF comparison, CBF
review, Data archive, restore, and delete, Configuration setup. System
includes xenon gas delivery system, software, computer, console,
network facility with optical disc, color printer and 300 liters of 80%
xenon/20% oxygen as well as installation, software updates, user
training, and one year warranty to include all parts and labor. System
by DDP, Inc. or equal. The following provisions and clauses shall
apply to this solicitation: FAR 52.212-1 Instructions To Offerors -
Commercial Items; FAR 52.212-2 Evaluation of Commercial Items; FAR
52.212-3 Offeror Representations and Certifications-Commercial Items;
FAR 52.212-4 Contract Terms and Conditions-Commercial Items; FAR
52.212-5 Contract Terms and Conditions Required to Implement Statutes
or Executive Orders-Commercial Items apply to this request. Pursuant to
52.212-2 The Government intends to make a single award to the
responsible offeror whose offer is the most advantageous to the
Government. Award will be based on (1)technical acceptability as
determined by the product literature submitted which demonstrates
ability to perform required functions. (2)past performance and
(3)price. The following clauses and or provisions are added by
addendum: The period of acceptance stated in FAR 52.212-1 is changed to
state that prices offered must be valid through December 31, 1996. VAAR
852.270-4 Commercial Advertising; VAAR 852.210-71 Guarantee; VAAR
852.210-77 Brand Name or Equal; VAAR 852.210-75 Technical Industry
Standards and; FAR 52.214-21 Descriptive Literature. Proposals shall
include all information required in 52.212-1 and a completed copy of
provision 52.212-3. Offers must be submitted to the attention of Jane
Ferencz(90C) at the VAMC 1201 NW 16th St., Miami, FL 33125. All offers
must be received not later than close of business (4:00 PM E.S.T.) on
9-24-96. Late offers will NOT be accepted. Discussion will be held
