combined synopsis for commercial items prepared in accordance with the
format in FAR subpart 12.6, as supplemented with additional
information included in this notice. This announcement constitutes the
only solicitation; solicitation number 549-98-96. This solicitation
action is for supplies or services for which the Government intends to
solicit and negotiate with only responsible small business sources
able to provide the Brand Name or equal product. Proposals are being
requested from SMALL BUSINESS ONLY and a written solicitation will not
be issued. The solicitation document and incorporated provisions and
clauses are those in effect through Federal Acquisition Circular 90-41.
The Standard Industrial Classification Code (SIC) is 5047. Prices for
Brand Name or Equal and descriptive literature shall be submitted as
follows for each item. ITEM I - One each Luxar ''LX-220'' Laser System.
Salient characteristics: System must be a carbon dioxide laser system,
portable - 40 lbs or less, have capability of flexible fiber delivery
for light, have scalpel-like feel. System must provide for: (i)
General Dentistry Applications: Restorative/Prosthetics; exposure of
subgingival caries with hemostasis, creation of dry field after removal
of defective amalgams or carious lesions, crown toughing in lieu of
retraction cord, soft tissue crown lengthening, creation of ovate
pontic sites. Periodontal; gingivectomy procedures indicated by
medication, infection, abscess or anatomical disorders, anterior
cosmetic gingivoplasty, distal wedge procedures, labial and lingual
frenectomies. Orthodontics; bracket exposure, treatment of bracket
hyperplasia. (ii) Surgery; sterilization of wound sites, incisional and
excisional biopsies, fribroma removal, removal of benigin - malignant
- hyperkeratotic and vascular lesions, treatment of aphthous ulcer,
coagulation of graft donor sites, establishment of graft recipient
sites, removal of mucoceles, initial incision for extraction, soft
tissue tuberosity reduction, vestibuloplasties and extensions,
operculectomies. Treatment of medically compromised patients;
hemorrhagic disorders, mucocitis due to chemotherapy, hyperlasia due to
hypertension medication. Treatment of Fernectomies; labial and lingual.
Implant Dentistry; second stage implant uncovering, maintenance of
tissue surrounding implants, uncovering for emergency treatment.
Cosmetic Facial Surgery; superficial telangiectasias, skin tags,
pigmented lesions, nevi. Laser Assisted Uvula-palatoplasty; treatment
of snoring cessation, treatment of sleep apnea. (Luxar product code
LX-20). ITEM II - Twenty-five each 10mm ''Long Ceramic'' .8 Spot (Luxar
product code 00609-01/LTX-C10). ITEM III - Twenty-five each 5mm ''Short
Ceramic'' .8 Spot (Luxar product code 00609-04/LTX-C5). ITEM IV - Ten
each 10mm mini ext ''Small Spot-Fine Tip'' .4 Spot (Luxar product code
00609-03/LTX-010SSD). ITEM V - Five each 19mm ext ''Extra Small Spot''
.3 Spot (Luxar product code 00609-05/LTX-010XSS). ITEM VI - Fifteen
each 40mm curved ext ''Long Curved'' .8 Spot (Luxar product code
00608-01/\LXT-040C). ITEM VII - Ten each 5mm ext ''Large Spot'' 1.4
Spot (Luxar product code 00628-01/LXT-005LS). ITEM VIII - Fifteen each
120mm straight .8 Spot (Luxar product code 00608-06/LXT-120ST). ITEM
IX - One each Turbinate Reduction Handpiece (Luxar product code
LXS-BD120-090). Preceding description represents the minimum
characteristics required. Offers shall be submitted with FOB
destination pricing and shall be delivered within 30 calendar days
after receipt of contract award and purchase order. At no additional
cost to the Government, the Contractor shall provide two each
user/operators manuals and two each maintenance manuals, and a one year
warranty to include parts and labor. The provisions at 52.212-1,
Instructions to Offerors - Commercial Items, applies to this
acquisition. The provisions at 52.212-2, Evaluation - Commercial Items,
applies to this acquisition. Proposals shall be evaluated on a
''Go-No-Go'' basis with price, technical criteria and contractor past
performance being of equal value. An award shall be made to the
responsible offeror whose offer conforming to the solicitation will be
the most advantageous to the Government. All offers are to include,
with their offer, a completed copy of the provisions at 52.212-3,
Offeror Representations and Certifications - Commercial Items (a copy
of this provision may be requested by FAX to (214) 372-7066 or by
