DINWOODY (706) 787-3894 (Site Code DADA08) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; offers are being requested and a written solicitation will not be issued. Solicitation number DADA08-96-R-0022 applies and is issued as an Request For Proposals (RFPs). This combined synopsis/solicitation and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 90-38 and Defense Acquisition Circular 91-10. The standard industrial classification code is 8011 and the business size is $3.5 Million. Contractor shall provide Gastroenterology Physician Services on site at Martin Army Community Hospital (MACH, Fort Benning, Georgia. Contractor shall provide inpatient and outpatient Gastroenterology Physician Services at Martin Army Community Hospital (MACH), Building 9200, Fort Benning, Georgia. Contractor shall provide these services to eligible beneficiaries of the military health care system. Healthcare services provided by the contractor shall be of a quality meeting or exceeding current recognized national standards. Services performed under this contract are personal in nature and the performance of the contract physician is subject to the day-to-day supervision that is comparable to that exercised over military and civil service healthcare providers engaged in comparable work. Any personal injury claims alleging negligence by the contract physician within the scope of the healthcare providers performance of this contract shall be processed by DoD the same as claims alleged against government personnel. Contractor shall perform all services in accordance with ethical, professional disciplines of the health field. Contractor shall perform all duties in such a manner that they meet all JCAHO Standards. Contractor shall comply with all procedures, policies and practices associated with Martin Army community Hospital, Fort Benning, GA. Contract physicians shall be of a quality meeting or exceeding current recognized national standards as established by the JCAHO. QUALIFICATIONS: Contract physicians providing services under this contract shall have the following qualifications: 1. be either board certified in Gastroenterology, or have successfully completed a Gastroenterology training in a United States approved fellowship; 2. continuously employed in the practice of Gastroenterology or in Gastroenterology training for two (2) years prior to the commencement of services under this contract; 3. current certification in Basic Life Support (BLS) and Advanced Cardiac Life Support (ACLS); 4. current controlled substances registration certificate from the Drug Enforcement Agency (DEA) in accordance with the laws of the state of Georgia; 5. completed the Occupational Safety and Health Administration (OSHA) Standard: Occupational Exposure to Bloodborne Pathogens and OSHA Tuberculosis Training; 6. credentialed by the Martin Army Community Hospital prior to commencement of services; 7. certification of a physical examination within the 45 days prior to commencement of services. PERFORMANCE PERIOD: This requirement is for a firm fixed price contract that includes a base year and four (4) one (1) year option periods with the option to extend services for up to six months after contract expiration. Offerors must submit pricing for the base year and each option period. Offers for the base year and each option periods can vary in price. ITEM 0001, Gastroenterology Physician Services, Base Year (1 Oct 96-30 Sep 97), Quantity 1410 Hours; ITEM 0002, Gastroenterology Physician Services, First Option Period (1 Oct 97-30 Sep 98), Quantity 1410 Hours; ITEM 0003, Gastroenterology Physician Services, Second Option Period (1 Oct 98-30 Sep 99), Quantity 1410 Hours; ITEM 0004, Gastroenterology Physician Services, Third Option Period (1 Oct 99-30 Sep 00), Quantity 1410 Hours; ITEM 0005, Gastroenterology Physician Services, Fourth Option Period (1 Oct 00-30 Sep 01), Quantity 1410 Hours. CREDENTIALING: All physicians providing services under the terms and conditions of this contract shall have their credentials reviewed by the Credentials Committee of the MTF and be granted hospital privileges by the MTF commander in accordance with AR 40-68 and local MEDDAC regulations prior to commencement of services. 2. Physician's curriculum vitaes and resumes shall be submitted by the contractor with their offer. 3. Should the contractor need to add additional physicians during the contract period, the same credentialing process shall apply for each additional physician. 4. The contractor must comply with requests for informal interviews with the contractor's physician(s). The credentials committee may request an informal interview over the telephone or present questions in writing to the contractor's physician. 5. Credentials packets are available upon request. HEALTH REQUIREMENTS: All contract physicians, including backup physicians, performing services under this contract shall comply with the all MTF health and immunization requirements. The expense for all physical examinations to comply with the health requirements shall be borne by the contractor at no additional cost to the government. HOURS OF PERFORMANCE: Contract Physician(s) shall perform four eight (8) hours a week on-site (4 hours in the clinic and 4 hours of procedures). Procedures will be performed on Monday, Wednesday and Friday or as scheduled by the Officer-In-Charge (OIC), Gastroenterology Clinic. Hours scheduled for shifts do not include travel time involved in reaching the clinic. No payment of overtime is authorized; however, the contract physician shall fulfill his/her professional obligation to the patient before the working day is completed. ON-CALL COVERAGE: Contract physicians shall provide emergency on-call coverage and telephone consultation seven (7) days per week, twenty-four (24) a day in blocks of time scheduled by the OIC of Gastroenterology Clinic and in rotation with other Gastroenterology providers at the MTF. While on-call, the contract physician is not required to remain at MACH or at his or her place of residence but must be available by contact pager during the time. He/she is required to respond telephonically within 10 minutes after receiving a call and must be prepared to provide personal consultation and/or assistance at the facility within 30 minutes, if requested. Routine clinic hours will not be required on Public/Federal holidays and MTF training holidays.
