a combined synopsis/solicitation for commercial services prepared in
accordance with the format in Subpart 12.6 as supplemented with
additional information included in this notice. This announcement
constitutes the only solicitaion. Proposals are being requested and a
written solicitaion will not be issued. THIS SOLICITATION IS ISSUED AS
A REQUEST FOR PROPOSAL. This procurement is utilizing the size
standard in FAR 19 under Standard Indsutrial Code 8071. POINT OF

ARE DUE ON OR BEFORE 27 SEP 96 AT 3:30 P.M. SEND ALL PROPOSALS TO: 37TH
CONTRACTING SQUADRON/LGCXC, ATTN: SUSAN EDWARDS, 1655 SELFRIGE AVENUE,
BLDG 5450, LACKLAND AFB TX 78236-5253. DESCRIPTION: The contractor
shall provide two mobile Mammography Units for women selected by
Wilford Hall Medical Center and The Wellness Clinic at Lackland AFB TX.
Screenings for breast cancer will be on approximately (CLIN 0001) 1000
women and will be held on days and times specified. The dates are as
follows: 9/28/96, 10/04/96, 10/14/96, 11/01/96,11/08/96, 11/09/96,
11/16/96, 12/06/96, 12/07/96, 12/09/96, 12/13/96, 12/14/96, 1/10/97,
1/11/97, 1/13/97, 1/18/97, 1/24/97, 1/27/97, 2/7/97, 2/8/97, 2/10/97,
2/21/97, 3/1/97, 3/3/97, 3/7/97, 3/15/97, 3/21/97, 4/4/97, 4/18/97,
4/26/97, 4/28/97, 5/2/97, 5/3/97, and 5/9/97. Women whose mammogram was
normal will receive two reports, one to keep and one to forward on to
their own charts. Also, in the event abnormal screening mammograms are
found, diagnostic mammography screens will be performed on
approximately (CLIN 0002) 200 women and ultrasounds will be performed
on approximately (CLIN 0003) 100 women selected by Wilford Hall Medical
Center and The Wellness Clinic at Lackland AFB TX. These studies wil be
dictated at contractor's Radiology location of convenience to the
patient. The abnormal mammography screens will be held on days and
times specified. The dates are as follows: 10/14/96, 11/8/96, 11/16/96,
12/9/96, 12/14/96, 1/13/97, 1/27/97, 3/21/97, 5/16/97, and 5/17/97. The
mobile Mammography Services shall consist of taking mammography films
and the professional interpretation of said films for patients who
present a written referral from Wilford Hall Medical Center or The
Wellness Clinic at Lackland AFB TX. Each patient will be required to
designate a primary care physician who will receive the mammography
report and who will conduct follow-up care for the patient. EFFECTIVE
DATE: The agreement is effective beginning September 28, 1996. It will
be the contractor's reponsibility to: a. Equipment will be dedicated
mammography equipment and meet appropriate radiologic standards for
mammography, such as the American College of Radiology's standards. b.
All technologists will be licensed by the State to perform
radiological procedures and certified as qualified to perform
radiological procedures by the American Registry of Radiologic
Technologists or their equivalent. c. Mammography films will be
interpreted by one of the contractor's Board Certified Radiologigists.
d. The contractor will comply with health care financing
administration interim final rules for mammography facilities. e. The
contractor will undergo an annual inspection and quality assurance
evaluation by a medical physicist. Documentation of such evaluation
shall be available for inspection. f. The contractor will undergo an
annual compliance inspection as scheduled by the Texas Department of
health Bureau of Radiation Control. g. The contractor will provide the
patient's designated provider with the written results of each
patient's mammography interpretation. h. The contractor will retain all
x-rays on location at one of its facilities per the license agreement
with the Bureau of Radiation Control. The contractor will provide the
mammography films to the appropriate physicians working with the
program, when so requested, with the understanding that the original
films must be returned to the contractor for storage. This solicitation
document and incorporated provisions and clauses are those in effect
through Federal Acquisition Circular 90-41. The provision at FAR
52.212-1, Instructions to Offerors-Commercial, applies to this
solicitaiton. The clause at 52-212-5, Contract Terms and Conditions
Required to Implement Statutes or Executive Orders-Commercial items,
applies to this solicitation. Specifically, the following cited clauses
are applicable to this solicitation: FAR 52.222-26, Equal Opportunity;
52.222-35, Affirmative action for special disabled and Vietnam era
veterans, 52.222-36, Affirmative action for handicapped workers;
52.222-37, Employment reports on special disabled veteran on the
Vietnam era, 52.222-41, Service Contract Act of 1965, as amended;
52.222-42, Statement of Equivalent rates for federal hires, in
compliance with the Service Contract Act of 1965, as amended and the
regulations of the Secretary of Labor, this clauses identifies the
classes of service employees expected to be employed under the contract
and states the wages and fringes benefits payable to each if they were
employed by the contracting agency subject to the provisions of 5 usc
5341 or 5332. Offerors must include with their offer a completed copy
of the provisions at FAR 52.212-3, Offerors Representation and
Certification-Commercial items. The clause at FAR 52.212-4, Contract
Terms and Conditions-Commercial items applies to this solicitation. The
clause at FAR 52.232-34, Optional Information for Electronic Funds
Transfer Payment items applies to this solicitation. Award will be
based on the lowest priced quotation from a responsible contractor in
accordance with FAR 9.104-1. Quotes may be submitted in writing or
