This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This is Solicitation 96-186 and is issued as a Request for Quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular Number 90-38. The procurement is UNRESTRICTED. The standard industrial classification code (SIC) is 3565. The small business size standard is not more than 500 employees. Description of Requirements For The Items To Be Acquired: Labeling/Tabbing Machine, One (1) Ea, with Shipping, Handling and Installation. All items shall be delivered within 30 days after the date of contract award. Deliveries shall be made, F.O.B. Destination, within consignees premises, to: Centers for Disease Control and Prevention/MASO, Publication Distribution Warehouse, 5665 New Peachtree Road, Atlanta, GA 30341. The FAR Provisions at 52.221-1, Instructions to Offerors. Commercial items, FAR Part 13.106-1, Evaluation of Quotes, and FAR 52.121-4, Contract Terms and Conditions. Commercial Items, apply to this acquisition. The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following elements should be addressed and they shall be used to evaluate offers: Descriptive literature of the machine. The following specifications are required of this equipment: (1) Vacuum TableTop - Should have; a Stepper Motor for precise and quick indexing; Fiber Optic Sensors for accurate and trouble-free label sensing; Flexible Design (fold can be to the right or the left for tabbing); Bridge reduced top sheet skewing; Quick-release lever to reduce jam removal time. (2) Electrical Requirements - 120VAC @ 50-60 Hz; (3) Maximum Operating Speeds - Single Tabbing - 20,000 pieces/hour; Double tabbing - 15,000 pieces/hour.
