Freeman (216) 838-6091 This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; This is the only notice of this requirement, a written solicitation will not be issued. Solicitation Number IFB 541-125-96 is issued as an Invitation For Bid (IFB). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 90-41. This is a Sole Source procurement under the Standard Industrial Classification (SIC) Code 5047 and Small Business Size Standard of 500 Employees, with Olympus America, Inc. for procurement of a File Manager Data Reporting, Image Recording and Storage System. This system shall consist of the following items, quantities and units of purchase: 1). 1 each - Endolink Network Server 25 user capability; 2). 1 each - Network Hub for up to 24 workstations; 3). 6 each - Image manager V6.0 Network Workstation GI/Pulm; 4). 1 each - Endolink Information Gateway; 5). 1 each - Netware Flex IP for Gateway; 6). 1 each - Deltis 10 Platter Optical Jukebox; 7). 1 each - Endolink Print/Archive Server; 8). 2 each - 13'' Color Monitor; 9). 1 each - Chartnote/GI 2 user reporting module; 10). Uninterrupted Power Supply; 11). 1 each - Internal UPS card for external UPS unit; 12). 2 each Hewlett Packard Laser JET 4 Plus with cable; 13). 2 each - Cable for DOT Matrix or Laser printer; 14). 6 each - 10 foot Digital File Video Cable; 15). 4 each - 10 foot Digital File Remote Cable; 16). 3 each - 9 pin to BNC, RGB, SYNC Cable; 17). 2 each - 10 foot 9 pin to 25 pin RS-232 cable; 18). 9 each - Harnessed RGB and SYNC cable 6 feet in length; 19). 1 each - In Line SYNC Attenuator for CV-100; 20). 8 each - Single Gang GBS-NTSC-AUX faceplate; 21). 6 each - Single GANG Network 10-Base-T faceplate; 22). 1 each - DG Central network station; 23). 40 each - Insulated BNC Connector for faceplate; 24). 12 each - RJ-45 1-Base-T faceplate connector CAT5; 25). 8 each - Y/C connector for faceplate; 26). 6 each - 10-Base-T patch cable 10 feet in length; 27). 8 each 10-Base-T patch cable 5 feet in length; 28). 1 each - Installation Charge. Contractor shall meet the following requirements: System shall be compatible with our existing Olympus Video Endoscopy Systems and our various Olympus endoscopes which consist of the following systems, EVIS 100, EVIS 130, EVIS 140 and EVIS 200. System shall have a proven track record for use with our existing upgraded Olympus Video system. System shall be compatible with and be able to interface with our Decentralized Hospital Computer Program (DHCP), and allow reporting from our local network system to DHCP. Contractor shall coordinate installation of system with the Contracting Officer's Technical Representative (COTR). Delivery shall be Free on Board (FOB) Destination, within 270 days of award. The provision at FAR 52.212-1, Instructions to Offerors-Commercial Items, applies to this acquisition with the following changes, items (d), (e) and (h) of the standard clause are deleted. Bidders shall include a completed copy of the provision at FAR 52.212-3, Offeror Representations and Certifications-Commercial Items, with their bid. The clause at FAR 52.212-4, Contract Terms and Conditions Commercial Items, applies to this acquisition. The clause at FAR 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders Commercial Items, applies to this acquisition which shall include the following clauses: 52.203-6, 52.203-10, 52.219-8, 52.219-14, 52.222-26, 52.222-35, 52,222-36, 52,222-37, and 52.225-3. Warranty of said equipment shall be in accordance with standard industry practices in effect at time of award. Interested readers may identify their interest and capability to meet the requirements by submitting a capability statement which must contain adequate information on supplying this type of system, proof of compatibility with Olympus systems, proof of ability to interface with DHCP and qualification to provide the product. This notice of intent is not a request for competitive bids.
