synopsis for commercial items prepared in accordance with the format
in FAR subpart 12.6, as supplemented with additional information
included in this notice. This annoumncement constitutes the only
solicitation; solicitation number 549-11-97. This solicitation action
is for supplies or services for which the Government intends to solicit
and procure from U.S. Medical Instruments or distributors for
''SafeSnap'' Retractable Needle Syringes; 1cc insulin, 1cc TB, 3cc, 5cc
and 10cc. This is not a notice for competitive proposals. The minimum
requiremnet is for the provision of a safety syringe where the used
needle is retracted into the barrel of the syringe. Interested persons
may identify their interest and capability to respond to the minimum
requirement by providing a bid to include descriptive literature. A
written solicitation will NOT be issued. Prices for each item shall be
submitted by CASE price for each item: Item I - 1cc Insulin Syringe,
28Ga x 1/2'' needle, 100/Bx (8 Bxs/Cs), US Medical Product No.
0128012-010, estimated usage for a year is 90 cases. Item II - 1cc TB
Syringe, 28Ga x 5/8'' needle, 100/Bx (8 Bxs/Cs), US Medical Product No.
0125058-020, estimated usage for a year is 105 cases. Item III - 3cc
Syringe, 22Ga x 1'' needle, 100/Bx (12 Bxs/Cs), US Medical Product No.
13711, estimated usage for a year is 96 cases. Item IV - 3cc Syringe,
24Ga x 5/8'' needle, 100/Bx (12 Bxs/Cs), US Medical Product No. 13720,
estimated usage for a year is 96 cases. Item V - 5cc Syringe, no
needle, 100/Bx (12 Bxs/Cs), US Medical Product No. 15700, estimated
usage for a year is 24 cases. Item VI - 10cc Syringe, no needle, 50/Bx
(12 Bxs/Cs), US Medical Product No. 10700, estimated usage for a year
is 114 cases. The estimated quantities given are for offeror's
information for bid preparation only and does not imply nor commit the
Government to purchase the estimated quantity under an awarded
contract. Delivery orders shall be placed against an awarded contract,
contract period date of award through 12 months or date of
termination, whichever is sooner. Orders shall be placed by the VA
Medical Center Dallas, Dallas, Texas 75216 and or the Sam Rayburn
Memorial Veterans Center, Bonham Texas 75418. Delivery shall be FOB
destination, within 15 and no more than 20 days after receipt of the
order, verbal or written, confirmation orders shall be issued at the
request of the Contractor. Should a National contract; NAC, GSA, etc be
issued during the period of the station contract, then the national
contract shall take precedence and the station contract shall be
terminated. The provisions at FAR 52.212-2, Instructions to Offerors -
Commercial Items, applies to this acquisition, All offerors are to
include, with their offer, a completed copy of the provisions at
52.212-3, ''offeror Representations and Certifications - Commercial
Items (a copy of this provision may be requested by FAX at (214)
372-7066. The following FAR clauses apply to this acquisition:
