(513)475-6339 This is a combined synopsis/solicitation for commercial
items prepared in accordance with the format in FAR Subpart 12.6, as
supplemented with additional information included in this notice. This
announcement constitutes the only solicitation; Invitation For Bids
(IFB) are being requested and a written solicitation will not be
issued. Solicitation document and incorporated provisions and clauses
are those in effect through Federal Acquisition Circular 34. The SIC
Code is 3571 and the small business size is 1000 employees. Contractor
shall provide, install and train personnel on six (6) voice dictation
systems with the following configuration: Pentium 200 Mhz system; not
less than a 200 Mhz CPU; 32MB RAM; 256KB Cache; 4GB Hard Drive; 3.5''
1.44MB Floppy Drive; Mini-tower case with 230 watt power supply;
Cooling fan for Pentium CPU; Intel ZAPPA PCI/ISA Bus Motherboard;
Hercules PCI Bus VGA Card with 2MB DRAM; BusLogic BT-930 Fast SCSI-II
PCI Controller card; 17'' Non-interlaced Touchscreen Monitor; 101 Key
keyboard; Microsoft Mouse; MS-DOS 6.22; Windows 3.1x; MS Word for
Windows 6.x. Each system shall include all hardware and software
required to operate and maintain the stand alone system and able to be
converted to a network environment. System must be capable of the
following: Dictating not less than 100 words per minute; 90% or greater
on average accuracy before final adaptation; 95% or greater on average
accuracy after adaptation; Vocabulary must include emergency room
muli-disciplinary with no less than 15000 words, to include history,
physical and disposition ability; System must be capable of batch
processing all dictations; Voice engine must be adaptable to individual
users accent and pronunciation, and be natural free flow continuous
speech recognition, with user training session time not to exceed sixty
minutes; Qty of two (2) form software compatible with voice dictation
system, have ability to accept multiple input devices, utilize existing
forms by scanning, voice navigation ability without training, and no
less than 20 form capability. Contractor shall deliver and install
three (3) systems each at the VA Medical Center Cincinnati and the VA
Outpatient Clinic Columbus, Ohio. Delivery shall be F.O.B. destination.
All cost associated with the purchase, delivery, installation, training
and adaptation of equipment shall be included in Contractors offer.
Delivery and installation shall be accomplished within sixty (60) days
after contract award. The following provisions apply to this
acquisition: 52.212-1 Instructions to Offerors--Commercial Items;
52.212-2 Evaluation--Commercial Items; 52.212-3 Offer Representations
and Certifications--Commercial Items; 52.212-4 Contract Terms and
Conditions--Commercial Items; 52.212-5 Contract Terms and Conditions to
Implement Statutes or Executive Orders--Commercial Items, including
clauses incorporated by reference under Section (b)(1) through (11) and
(15). Bidders responding to this announcement shall submit a unit price
for each system, along with the information required in 52.212-1, and
a completed copy of provision 52.212-3 by September 27, 1996, 2:00 p.m.
