H -- TESTING LABORATORY SERVICES FOR AMB. CARE ADDITION CONSTRUCTION SOL RFP 644-1-97 DUE 111496 POC Kathy Bernal, (602) 222 6404 Testing Laboratory Services to inspect materials, workmanship and perform tests as required during site prep and construction work, for Phase 1 Ambulatory Care Addition at the VAMC Phoenix. Minimum qualifications require principals of testing lab to be registered professional engineers with not less than five years experience in the various type of testing required under this contract. Each person in charge of laboratory testing, field testing and inspection services must have not less than one year full time experience in the duties to be performed under this contract. Services to include inspection, testing, written and verbal reporting and providing published standards as applicable. Types of testing to be provided (not all inclusive) are pavement subgrade-compaction-aggregate base course-asphaltic concrete pavement, site utility trenches, site concrete, slump testing, and materials testing such as concrete cylinder tests,masonry mortar, grout and unit tests as well as welding and bolting type inspections. Contract duration to be consistent with construction estimated at 580 calendar days but will require provisions for contract extensions if required. (0262)
