65 -- GASTROINTESTINAL VIDEOSCOPE SOL 583-112-96 DUE 100396 POC Vickie Ruggles, Contracting Officer This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation No. 583-112-96 is issued as a request for quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 90-42. Contractor to provide one Gastrointestinal videoscope to include the following: Model GIF-XQ140 with 9.4 mm insertion tube diameter, 2.8mm channel diameter, including standard accessories and carrying case. Contractor shall also provide Trade-IN of two (2) Gastroscopes, Model GIFIT20 and Model GIF100, two (2) operator manuals, one (1) service manual, one (1) schematics, and one (1) parts list and ordering information. System shall be brand name or equal to the equipment supplied by Olympus America Inc., 2080 Springer Drive, Lombard, IL 60148. Brand name or equal items may be considered if descriptive literature is provided with proposal. Delivery will be made within 45 days from receipt of order, which will state specific delivery point in Indianapolis, 46222. The provision at 52.212-1, Instructions to Offerors - Commercial, applies to this acquisition. 52.212-2, Evaluation - Commercial Item applies to this acquisition. The following factors shall be used to evaluate offers: (1) price, (2) warranty, (3) delivery time, and (4) technical (capability of the offered system to meet agency needs). Offerors shall include a completed copy of the provision at 52.212-3, Offerors representations and Certifications - Commercial Items, with its offer. 52.212-4, Contract Terms and Conditions - Commercial Items, 52.214-21, Descriptive Literature, 852.210-77 Brand Name or Equal and 52.232-33, Mandatory Information for Electronic Funds Transfer Payment, applies to this solicitation.
