65 -- SURGICAL GUIDANCE SYSTEM SOL 583-115-96 DUE 100396 POC Vickie Ruggles, Contracting Officer This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation No. 583-115-96 is issued as a request for quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 90-42. Contractor to provide one C-Trak Dual Probe Surgical Guidance System to include the following: Indium Probe with Collimator, Technetium Probe with Collimator, C-Trak Radiation Blocking Plate, Printer and Cable, Foot Switch, Calibration Source, Batteries, Two Carrying Cases. Contractor shall also provide two (2) operating manuals, one (1) service manual, one (1) schematics, and one (1) parts list and ordering information. System shall be brand name or equal to the equipment supplied by Coffey Medical, Inc., 2209 Pebblebeach Drive, Carmel, IN 46032. Brand name or equal items may be considered if descriptive literature is provided with proposal. Delivery will be made within 45 days from receipt of order which will state specific delivery point in Indianapolis, 46222. The provision at 52.212-1, Instructions to Offerors - Commercial, applies to this acquisition. 52.212-2, Evaluation - Commercial Items applies to this acquisition. The following factors shall be used to evaluate offers: (1) price, (2) warranty, (3) delivery time, and (4) technical (capability of the offered system to meet agency needs). Offerors shall include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications - Commercial Items, with its offer. 52.212-4, Contract Terms and Conditions - Commercial Items and 52.232-33, Mandatory Information for Electronic Funds Transfer Payment, applies to this solicitation. Clause 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders - Commercial Items, applies to this acquisition, including 52.233-3, 52.222-26, 52.222-35, 52.222-36, and 52.222-37.
