This announcement constitutes the only solicitation; proposals are being requested as a written solicitation will not be issued. The combined synopsis/solicitation number is DACA87- 96-R-0048 and is issued as a Requet for Proposal (RFP). The combined synopsis/solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 90-42. The Standard Industrial Classification (SIC) is 3841 and the Small Busines Size Standard is 500. Full and open competition is invited for a complete turnkey Nuclear Medicine Suite, including limited facility modification, at Nellis Federal Hospital, Las Vegas, Nevada. The system specification requires a DUAL DETECTOR GAMMA CAMERA; ADAC vertex or equivalent which should be optimal for one camera department to provide maximal patient throughout while reducing patient imaging time. The camera shall include two rectangular digital detector heads capable of rotating from 180 degrees to 90 degreees; minimum 3/8 inch thick sodium Iodid Crystal; minimum 20 inch by 15 inch detector surface; linear gain photo-mulitplier tubes; real time energy; linearity and uniformity correction; minimum three height analyzers; GANTRY: Dual ring configuration; programmabble positions; dual detector patient contouring for both SPECT and whole body imaging; 380 degree SPECT; variable height patient imaging table, 500lb table weight capacity; ability to image patient in upright position with single head; persistence scope located on acquisition cart which can be positioned next to the gantry; IMAGE PROCESSOR (2 each): 110-155 V 30 Amp; UNIX based operating system; 32-bit or greater RISC central Processing Unit (CPU); mass Storage media shall be a minimum 2 Gbyte Hard Drive; 150 Mbytes or greater Magnetic Tape Drive; 1.44 Mbytes 3.5'' floppy Drive; Read/writable 1.3 Gbyte Optical Disc drive; Graphic Accelerator that allows for 3-D imaging SPECT image reconstruction for diagnostic purposes including a 19'' color monitor,; 256 gray levels; 16 million colors; high speed modern; software shall be interfile Version 3.3 or higher;; 3-D Cardiac reconstruction; Cequal Cardiac Analysis software; Standard SPECT reconstruction; user Programmable Macros; ACCESSORIES: Include EKG Gate that provides display of ECG and R wave trigger for the acquisition of gated cardiac procedures; EKG Strip Recorder that provides hard copy of patient's ECG during gated procedures to insure accurate reporting of results and validation of image data; Uninteruptable Power Supply; 5KVA- 5KW; provides 6-16 minutes of power backup in the event of power failure; Collimators (pairs) that are low Energy (140keV) High Resolution; Medium Energy (300 KeV) General purpose; Low Energy (140keV) General purpose; Automatic Collimator exchange system; TRAINING shall be a minimum 5 days of system application on site with the first 2 days a weekend when no patients are schedules; Advanced protocol class at vendors location with tuition fully paid by the vendor; 30-day follow-up (after initial training) on site training; nuclear cardiology 3-day fellowship for physicians (tuition paid by contractor); laser interface for Kodak 2180 laser printer located in CT area of radiology and installed by the contractor; QC materials, bar phantom and SPECT resolution phantom; Furniture shall be tables and/or computer desks for image processing systems will be provided by the vendor; ADDITIONAL EQUIPMENT REQUIREMENTS: Dose Calibrator (Biodex Catalog #086- 270; Survey Meter (Biodex Catalog #051-014); 3cc Syringe Shield (Biodex Catalog # 007-735); Dose Calibrator Reference Sources (Cs 137-Biodex Catalog #101-356, Co 57 5 mCi (Biodex Catalog # 063-261); Radioisotope Storage Module (Biodex Catalog # 234-801); Decay Module (Biodex Catalog # 234-203); Sink and Waste Module (Biodex Catalog # 234-102); Continuous Countertop for the above (3) modules (Biodex Catalog # 234-999); INSTALLATION shall be performed by the manufacturer, all cables will be run in conduits; system shall meet NEMA standards before first patient usage; FACILTIITY MODIFICATION: Contractor shall provide complete set of architectural drawings in autoCad release 12 prepared by a license architect; provide electrical engineering as required to provide power per manufacturer specifications by a licensed engineer; provide structural engineering and floor loading calculations prepared by licensed engineer; provide mechanical engineering prepared by licensed enginer as required; provide and pay all cost for blue printing services; provide and coordinate all testing and inspection requirements in accordance with USAF regulations and Base Civil Enginering requirments; provide all barricades, dust barriers and temporary fencing required by standard paractices; temporary utilities will be provided by the hospital for the duration of the project; Approximately 16 lf of existing partition wall shall be removed; approximately 80 sq. ft of gypsum ceiling wil be removed; approximately 60 sq.ft. of 2'x 2'' ceramic floor tile will be removed in the existing toilet to provide space for gantry room; approximately 10 lf of countertop will be removed and returned to the government; Contractor shall furnish trash dumpsters; cordinate and schedule concrete cutting operations during off hours; provide cleaning of the suite at the end of the work day and end of the project; furnish all materials and labor to install toilet valve, sink and Type M copper condensate drain line for Fan Coil Unit (HVAC); Relocate existing and add new heads where needed to modify the wet-pipe fire sprinkler system for the affected areas to conform to the NFPA requirements; new heads will be Grinnell model F950 recessed pendant sprinkler heads with Grinnel model F944 escutcheon as required; furnish all labor and material to provide power to the new equipment (two 30 amp 208v and one 15 amp 115v ckts to serve power requirements for proposed equipment; furnish and install all electrical conduits, light fixtures, switches and outlets, 2 telephone outlets boxes with conduit and pull wire stubbed to ceiling space and fire stopping for conduit penetrations; Furnish and install one new Trane or equivalent, commercial grade, 3 ton chilled water fan coil A/C unit, all pipes, ducts and conduits shall be supported and braced per the SMACNA, furnish and install ventilation and exhaust, complete air balance; fabricate and install all structural steel support beams as required for supporting the equipment; install metal backing in walls for anchorage of wall mounted cabinetry; furnish and install approximately 64lf of interior wall partitions; approximately 40 lf of 1/16'' equivalent lead lined gypsum on two sides; install approximately 100 sq fet of new suspended drywall ceiling; install double boxed 16 gauge steel king studs and headers at all door frames; one 90 minute 4'x7' door, and one 4'x7' 90 minute door frame must be provided and installed in the corridor. All existing doors were provided by The Hallgran Company, Las Vegas, NV; relocate two existing doors; contractor shall provide frame and hardware for new 20 minute fire RH door; furnish and install approximately 400 sq ft new suspended T-Bar ceiling grid and acoustical lay-in panels which shall be Donn 24x-2'x4' standard grid with Armstrong #769A Cortega lay-in panels, both in manufacturer's standard white; furnish and install a new hardware package to match hospital standards at all new consisting of a Sergeant lever handle 18-81 or equal, hinges, smoke seal, door stop and Norton 8501 closer or equal; remove and reinstall two rows of high density Radiology films files in film storage room; Paint approximately 1,500 sq.ft using Sherwin Williams SW 1072 ''illusive Fawn'' latex semi-gloss enamel; provide and install approximately 250 sq. ft. of 2''x2'' of ceramic wall tile (Dal-tile brand number 1225 Crystalex Pat D CT Dove) and approximately 50 sq.ft of 2'x2' ceramic floor tile (Dal-tile brand, number DK316 coco brown; furnish and install approximately 200 sqft of seemless sheet Armstrong Classic Corion or equal vinyl flooring in hot lab; furnish and install approximately 250 sq ft of Armstrong Excelon or equal vinyl composite Tile; and provide a full time superintendent to oversee all aspects of the project. Actual field measurements will be done upon contract award. Period of performance is from award through December 20, 1996. Proposals shall be submitted on a firm fixed price basis. PAYMENT will be made only after a completed job is acceptable to the government.
