Montgomery, Regional Contract Specialist, 606-253-8880. The Federal
Bureau of Prisons intends to award a Non personal services contract for
an Optomertrist. Duties are to be performed in the Health Services
Unit, at the Federal Correctional Institution, Petersburg, Virginia.
The services include examination, diagnoses, and making of
recommedations to the Clinical Director for appropriate optometric
care/further treatment of ocular disease(s), prescribe eyewear, and any
medicallly indicated features of eyewear for patient's unique ocular
needs, perform tonometries and slit lamp examinations when medically
indicated/requested, and making necessary adjustments of institution
glasses/frames dispensed. The contractor shall also make recommedations
to the Health Services Administrator and Clinical Director for
treatment of eye diseases and the procurement of eyewear. The
contractor shall provide written reports of his/her clinical findings
and recommendations for treatment. The provider shall provide
continuing medical education to the medical staff as required and
scheduled by the Health Services Administrator. At a minimum, the
contractor must have a degree from an accredited college or university
in the field of Optometry. The potential candidate must submit a
Curiculum vitae as part of the bid package. This contract will include
a base year not to exceed (12) twelve months from date of award, with
four (4), one (1) year option periods for renewal. Estimated number of
sessions for the base year = 24, (two (2) sessions per month, one (1)
session equals four (4) hours in length.) Option Year #1 estimated
sessions = 24; Option Year #2 estimated sessions = 24; Option Year #3
estimated sessions = 24; Option Year #4 estimated sessions = 24.
Solicitation packages will be mailed approximately October 11, 1996,
with an approximate due date of November 1, 1996. It is anticipated
this solicitation will result from a Firm Fixed Price, Requirements,
type contract. The proposed contract is 100% set aside for small
business concerns. All responsible sources may submit an offer which
