Z -- OPEN END CONTRACT - ASBESTOS IH & AIR MONITORING SERVICES SOL IFB SDWT 630-01-97 DUE 111897 POC Franklin Bell Provide all labor, materials, equipment, personnel and supervision necessary to perform asbestos industrial hygiene assessment and air monitoring services on various in-house projects at this medical center. Contract is for period of (1) year, w/option to extend (2) additional years. Contract is on a unit price basis with estimated quantities and open-ended with individual work orders placed against, not exceeding $25,000 each, with the cumulative total of contract including option years, not to exceed $300,000. Issue date on or about 10/18/96. Bid opening date 11/18/96. This requirement is SET-ASIDE FOR EMERGING SMALL BUSINESSES WITHIN A 25 MILE RADIUS of this medical center. There is NON-REFUNDABLE FEE IN THE AMOUNT OF $50.00 TO BE SUBMITTED ONLY IN THE FORM OF A CERTIFIED BANK CHECK OR MONEY ORDER, PAYABLE TO THE DEPARTMENT OF VETERANS AFFAIRS.
