Your request must include complete company name, mailing address, CAGE code, phone and fax numbers, and company contact. RFQ willbe issued on 10/31/96 and close on 11/15/96. Requirement is for contractor to provide consultation services to employees and eligible dependents of the Naval Air Station, North Island, and tenant commands under CINCPACFLT. The range of problemsinclude, but are not limited to, alcoholism, drug abuse, emotional and family stress, financial, legal, and marital problems. Coverage will include approximately 2,644 employees plus eligible dependents, with approximately 100-115 referrals per year.Resulting contract will be for one base year and two options years, for a total of three years. Services will commence upon award (estimated to begin 12/1/96) and will be conducted within San Diego County limits. Contractor will be required to providea toll-free telephone number and a telephone answering service for making appointments on a 24-hour per day, 7-day per week basis.
