This system shall include the equipment, ancillary supplies and support services for the VA to perform the following laboratory test: Item 0001-Free Thyroxine (FT4); Item 0002-Thyroid Stimulating Hormone (TSH); Item 0003-Vitamin B12; Item 0004-Folate; Item 0005-Ferritin; Item 0006-Prostate Specific Antigen (PSA); Item 0007-Creatine Kinase/MB Fraction (CK-MB); Item 0008-Phenobarbitol; Item 0009-Gentamicin. Supplies and services are required for a basic year period commencing December 15, 1996 (or date of award whichever comes later) through September 30, 1997, with two option years. Supplies and services shall be delivered to the VA Medical Center, Cincinnati, Ohio via F.O.B. Destination.
