This announcement constitutes the only solicitation. A quote is being requested and a written solicitation will not be issued. Solicitation number F22600-97-T0408 is issued as a Request for Quotation (RFQ). The standard classification code is 5047, small business size standard is 100 employees. The incorporated document and provisions and clauses are those in effect through Federal Acquisition Circular 90-42. Contract Line Item 0001 is for 1 each Autoinjector, nonferrous, MRI Autoinjector used for precisely timed contrast administration for dynamic and functional imaging studies. Medrad Model SMR 200. System includes pedestal head mount and standard accessories. Also includes additional accessories, 2 CS MRI Disp Kit P/N SQK65, Medrad assurance plan and installation/application. Training will be provided for approximately 12 radiologists and 8 technicians. Two copies of the following shall be included with the equipment: Operator's Manual, Service Manual, Wiring Diagrams, Electrical Schematics, Spare Parts List, and Calibrating Procedures. Brand name or equal. Delivery terms shall be FOB Destination, with delivery to be made to 81 Medical Group/SGCS, Bldg 0468, 5th Street, Keesler AFB, MS 39534. Delivery shall be made within 60 days after award of contract. Inspection and acceptance shall be made at destination. Offers must be signed and may be submitted on your company letterhead or stationary, or your standard company quotation forms.
