Design and construction administration services for the renovation and
expansion of an existing ambulatory health care facility at
Dzilth-No-O-Dith-Hle, Huerfano, New Mexico. The total area included in
this project is approximately 1,276 square meters. This fixed price
type contract will require architectural and engineering services for
studies, designs, and construction services. The estimated construction
cost is to be between $1 and $4 million. The total project is to be
completed within an estimated 24 months after receipt of award. In
addition, we may add, the design of between 1 and 170 permanent
quarters to be located at Ft. Defiance, Arizona, during the course of
this contract by way of a contract modification. This project's
construction cost is estimated to be between $30 and $40 million. The
evaluation factors for this A/E selection are listed below in
descending order of priority with evaluation factors (1) and (2) being
of equal point value and items (4) and (5) being of equal point value:
(1) PROFESSIONAL QUALIFICATIONS OF FIRM AND ALL CONSULTANTS -
Professional qualifications and experience of firm for new design and
renovation of ambulatory health care facilities; length of time firm
has been established; (2) SPECIALIZED EXPERIENCE OF PROJECT RELATED
PERSONNEL - Experience and professional qualifications of key staff
assigned to this project; specific design ability and experience in
ambulatory health care facilities; (3) LOCATION OF MAIN OFFICE OF
FIRM(S) AND CONSULTANTS - Relative travel time and communication
ability between the prime firm and consultants relative to each other
and to the project location; knowledge of the locality of the project;
(4) OVERALL PERFORMANCE RECORD - Exhibited design abilities on
previous projects; managing; scheduling; budgeting; estimating; etc.
(5) DESIGN EXPERIENCE IN NATIVE AMERICAN CULTURES - Specific experience
and/or plan for addressing Navajo culture; (6) ANALYSIS OF CURRENT AND
PROJECTED WORKLOAD -demonstrate ability and maintain a total project
team to complete the project on schedule. THE PROPOSED CONTRACT LISTED
HEREIN IS SET-ASIDE UNDER THE BUY INDIAN ACT FOR 51 PERCENT
INDIAN-OWNED, CONTROLLED, AND OPERATED FIRMS. NOTE: In accordance with
Public Health Service Acquisition Regulations (PHSAR) paragraph
380.503(e), nor more than 50-percent of the work to be performed under
the prime contract award pursuant to the Buy Indian act shall be
subcontracted to other than Indian firms. For this purpose, work to be
performed does not include the provision of materials, supplies, or
equipment. Joint Ventures will be considered providing the Indian firm
is the managing entity and the Joint venture is at least 51 percent
Indian owned, controlled and operated (must be stated in the letter of
interest). Offerors who are currently certified by the Indian Health
Service (IHS) or the Bureau of Indian Affairs (BIA) as an ``Indian
Firm'' need to submit this certification along with their SF 254/255.
Offerors who are not currently certified by IHS or BIA are required to
submit proof (certification of tribal affiliation and degree of Indian
blood issued by BIA is acceptable proof) that the owner meets the
definition of ``Indian''. All responsible firms may submit an SF 254
and an SF 255 which shall be considered by the agency. The firm shall
submit: 1) One (1) Standard form 254, A/E Related Services
Questionnaire for the firm and all consultants; 2) One (1) Standard
Form 255, A/E Related Service for Specific Projects; and 3) A letter of
interest and a written certification that the firm is at least 51
percent Indian owned, controlled and operated, along with the
certification noted above. Submittals must be received by 4:30 pm.,
