McNab, 406/329-3766 This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number is R1-16-97-131Q and is issued as a request for quotation. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 90-42. This procurement is 100 percent set aside for small business, the standard industrial classification code (SIC) 8999, and the small business size standard is S5.0 million. Description of Requirements - This procurement is for rental of fully operated heavy equipment on an hourly basis. Equipment use and operation will be directed by the Government through the Contractor or designated representative. The Contractor shall also provide all operating supplies and support required for equipment operation. The work to be done entails root wad revetments along creek banks, excavation and regrading of the stream channel, and other work as directed. The start work date is December 2, 1996, and contract time is 50 days. The project site, Clear Creek on the Plains Ranger District, is located at T 49 N and R 30 W and is accessed by Forest Development Road 153, which intersects Forest Highway 7, Prospect Creek at milepost 210. Items To Be Acquired are - Line Item 01 Excavator, with thumb, 90-165 HP, Estimated Quantity 200 Hours; Line Item 02 Excavator, with thumb, 90-165 HP, Est. Qty. 200 Hours; Line Item 03 End Dump, 10-12 CY, Est. Qty. 160 Hours; Line Item 04 Equipment Transport, Est. Qty. 10 Hours. All equipment shall be in good mechanical and operating condition and shall be operated by competent operators experienced in the type of work for which the equipment is being used. Offerors should submit the type, make, model, year, horse power, and capacity for each piece of equipment in the quotation. List also qualifications of equipment operators such as special skills, training, or certifications. The FAR provision at 52.212-1, Instructions to Offerors-Commercial Items (Deviation for Simplified Acquisitions, MAR 1996) applies to this acquisition. Offers should be submitted on letterhead stationery and include the minimum information required under this provision. Offerors may request copies of the full text of referenced clauses at the contact point shown below. The provision at 52.212-2, Evaluation-Commercial Items (OCT 1995), is not applicable. However, quotations received will be subject to the Simplified Acquisitions Procedures, Part 13 of the Federal Acquisition Regulations, and may be evaluated using price and other factors, such as successful past performance or a demonstrated current ability to perform successfully. A completed copy of the provision at 52.212-3, Offeror Representations and Certifications-Commercial Items (JUN 1996), should be submitted with your offer. The clause at 52.212-4, Contract Terms and Conditions-Commercial Items (AUG 1996), applies to this acquisition. The clause at 52.212-5, Contract Terms and Conditions Required To Implement Statutes or Executive Orders-Commercial Items (Deviation for Simplified Acquisitions, MAR 1996), applies to this acquisition. Specifically, the following FAR clauses cited are applicable to this solicitation: 52.222-26, Equal Opportunity (APR 1984); 52.222-35, Affirmative Action for Special Disabled and Vietnam Era Veterans (APR 1984); 52.222-36 Affirmative Action for Handicapped Workers (APR 1984); and 52.222-37 Employment Reports on Special Disabled Veterans and Veterans of the Vietnam Era (JAN 1988); and 52.222-41, Service Contract Act of 1965, as Amended (MAY 1989). Service Wage Determination 76-1342 Rev. 18 is applicable. Consistent with customary commercial practices, FAR Clause 52.236-7, Permits and Responsibilities (NOV 1991); AGAR Clause 452.236-72, Use of Premises (FEB 1988); and FSAR Clause, Loss, Damage, or Destruction, also apply. Because the project work site is in the creek, pollution abatement practices must be observed.
