Provide Dental Hygiene Services for the U. S. Coast Guard Air Station Clinic, Cape Cod, Ma on a routine, part-time basis, Monday through Thursday seven and half (7-1/2) hours per day and Fridays four (4) hours per day, to all beneficiaries referred by the professional staff of the Air Station Clinic. All services shall be performed in accordance with the Statement of Work and all other terms and conditions of the solicitation. All interested offerors shall possess and maintain for the duration of the contract a current unrestricted license for the practice of Dental Hygiene in the State of Massachusetts. The contract performance period will begin approximately 01 April 1997 or date of award, whichever occurs later, through 30 September 1997 with four (4) one-year options. The SIC Code is 8011 with $5 million or less average annual receipts for small businesses within the last three (3) years. This solicitation will be issued on or about December 2, 1996.
