65 -- MOBILE CT SYSTEM SOL 673-43-97 DUE 120296 POC Eric Guidash 813-972-7588 This is a combined synopsis for commercial items prepared in accordance with the subparts in Federal Acquisition Regulation 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This solicitation is issued as a request for proposal. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 90-42. The Standard Industrial Classification Code is 5047. Pricing shall be submitted for rental of one (1) mobile CT System to include a laser camera for a period of six (6) months. Pricing shall also include delivery to and from customer site(Orlando, Florida), set-up of equipment at customer prepared site, orientation of the CT by the manufacturer's service engineer and full service maintenance for rental period to include x-ray tubes and all glassware. Offerors shall provide a completed set of provisions with proposal found at 52.212-3, Offeror Representations and Certifications - Commercial Items. Provisions and clauses found at 52.212-1, 52.212-4, 52.212-5 and 852.210-77 also apply.
