The Federal Bureau of Prisons is requesting Otorhinolaryngology (ENT) services for the United States Penitentiary, Route 5, Little Grassy Road, Marion, IL 62959. The Otorhinolaryngoloist (ENT) Contractor will be required to come to the Health Services Unit of both the United States Penitentiary and the Federal Prison Camp to provide diagnostic services and recommendations regarding treatment. Emergency and Telephonic Consultations will be performed by the contractor as deemed necessary by the Health Services Staff. This contract shall contain options for a base year (date of award to 9/30/97) and four one year option periods. Quotes must provide separate prices for the base year, option period one, two, three, and four.
