J -- TERM CONTRACT FOR SERVICE & MAINTENANCE OF PICKER PQ-200 CT SCAN SOL RFP 527-16-97 DUE 011097 POC Mrs. Jo Ann Petrillo, Contract Specialist, FAX 718 630-3558 Term contract to provide service and preventive maintenance for Picker PQ-200 CT SCAN Site#73972 located at the Brooklyn VA Medical Center. The following equipment options are included: Hyperlan Data Link - VOXEL Q with one input optical disk - ZAP-100 spiral nose remote diagnostic station - glassware replacement is also included. Services shall include diagnostic, calibration, repairs and preventive maintenance. Also included is the assessment of liquidated damages for loss of production due to unit downtime. Repairs shall only be made by manufacturer trained technicians. Weekly preventive maintenance visits will be required. The initial term of the contract shall be from February 1, 1997 through September 30, 1997 with a Government option to extend the term of the contract for two (2) additional one year periods.
