This is a combined synopsis/RFQ for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in the notice. This notice constitutes the only RFQ: proposals are being requested and ''written RFQ'' will not be issued. RFQ 502-12-97 is issued as a request for proposal and incorporates provisions and clauses that are in effect through Federal Acquisition Circular 40. Standard Industrial Classification (SIC) code for this acquisition is 5047. Contractor shall provide item #1 Scanning Laser Tonograph for optic nerve head imaging and measurement that will digitize the optic nerve head and is compatible with DHCP and will follow optic nerve progression through the computer. The system must provide dependable data for the diagnosis and management of glaucoma with the optic nerve head imaging and measurements with enhanced image visualization, volumetric analysis, baseline and comparison images acquired through undilated pupils. The software will operate on microsoft windows platform and system will include a color printer. Deliver FOB destination 30 days after award date. Standard Industrial Classification Code (SIC) 5047, computer, peripheral equipment and software shall apply to this procurement.
