This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation number F22600-97-Q5074 is issued as a request for quotation (RFQ). The incorporated document and provisions and clauses are those in effect through Federal Acquisition Circular 90-42. This action is being considered for 100% set aside for small business (SB) concerns. The standard classification code is 7359, small business standard is $5.0M. Contract line item 0001 is to lease one each Mobile Treatment Planning Simulator for the Radiation Oncology Clinic, Keesler Medical Center, Keesler AFB, MS, for a period of four (4) months, beginning 15 Jan 97 and continuing through 14 May 97. Required services include: installation and set up of mobile Treatment Planning Simulator; provide training on operation of equipment to Keesler personnel; provide a preventative maintenance inspection upon installation; perform all tests, adjustments, lubrication, etc., in accordance with manufacturer's maintenance guide; respond on site within 24 hours of notification of equipment problems (0700-1700 hours weekdays); complete repair within 24 hours of arrival; and take down and removal of equipment at completion of performance period. Required mobile simulator specifications include: Geometry--Gantry rotation 360 degrees, Couch rotation plus or minus 15 degrees from zero; X-ray tube--Peak potential 125kVp, Fluoro (ABC); Power Requirements--Voltage 208V; Lasers--minimum 5; Anticollision Interlocks; Collimator rotation plus or minus 90 degrees from zero; and minimum two fluoro monitors, one at control station, one in simulator room. Delivery terms shall be FOB Destination, with delivery to be made to 81 Medical Group/SGSL, Bldg 0468, 5th Street, Keesler AFB, MS 39534. Delivery shall be made so that usage can begin 15 Jan 97. Inspection and acceptance shall be made at destination. The following clauses and provisions are incorporated and will remain in full force in any resultant contract: FAR 52.212-1, Instructions to Offerors-Commercial Items (Oct 1995), is incorporated by reference and applies to this acquisition, and para (b) is amended to read: ''Submit signed and dated offers to 81 CONS/LGCV, 200 Fifth Street, Room 104, Keesler AFB, MS 39534-2102, at or before 3:00 p.m. CST, 18 Dec 96. Offers must be signed and may be submitted on your company letterhead or stationary, or your standard company quotation forms.'' Clause 52.212-2, Evaluation-Commercial Items (Oct 1995)--Evaluation Factors are past performance, product features and capabilities, and price. Offerors shall submit product literature for evaluation purposes. The evaluation factors will be evaluated as being substantially equal.
