commercial items prepared in accordance with the format in FAR Subpart
12.6, as supplemented with additional information included in this
notice. This announcement constitutes the only solicitation; A Request
For Proposals (RFP) is being requested and a written solicitation will
not be issued. Solicitation document and incorporated provisions and
clauses are those in effect through Federal Acquisition Circular 34. It
is the intent of the VA Medical Center, Cincinnati, OH, to enter into
a sole source contract under the authority of 41 U.S.C. 253(c)(1), with
Micromedix, Inc., of Dayton, OH, to provide a Computerized Clinical
Information System for the VA Medical Centers in Cincinnati, Columbus,
Dayton, Chillicothe, and Cleveland, Ohio. Services are required for a
basic year commencing date of contract award through September 30,
1997 and four subsequent option years. The required database is
available from only one responsible source and no other type of service
will satisfy agency requirements. The database shall include
comprehensive writings in respective subject areas as follows: Item
0001-Identification of potentially toxic material in commercial,
pharmaceutical, and biologic substances; Item 0002-Tablet and capsule
identification for U.S. and foreign ethical and OTC pharmaceutical
preparations; Item 0003-Drug information system containing
comprehensive monographs on drugs in therapy; Item 0004-Pharmacopoeia,
licensed from the Pharmaceutical Society of Great Britain and provides
information on foreign pharmaceutical products; Item 0005-Toxicology,
Occupational Medicine, and Environmental Series; Item 0006-Information
for diagnosis and therapy in a broad spectrum of medical areas; Item
0007-Discharge instructions for disease and trauma states, and patient
instructions for medications. The information system is required on
computer tapes for mainframe computers and must include access
software. Delivery shall be F.O.B. Destination to each of the
facilities identified. The following provisions apply to this
acquisition: 52.212-1 Instructions to Offerors--Commercial Items;
52.212-2 Evaluation--Commercial Items; 52.212-3 Offer Representations
and Certifications--Commercial Items; 52.212-4 Contract Terms and
Conditions--Commercial Items; 52.212-5 Contract Terms and Conditions to
Implement Statutes or Executive Orders--Commercial Items, including
clauses incorporated by reference under Section (b)(1) through (11) and
(15); 52.217-8 Option to Extend Services; and 52.217-9 Option to Extend
the Term of the Contract with one day written notice given and contract
duration not exceeding 5 years, 6 months. The Offeror shall submit a
yearly price for the basic year and four one-year option periods, along
with the information required in 52.212-1, and a completed copy of
provision 52.212-3 by December 12, 1996, 2:00 p.m. local time. Services
beyond September 30, 1997, 98, 99, 2000 and 2001 are contingent upon
