The combination synopsis/IFB initially published on 11/5/96 and amended to indefinitely extend bid opening on 11/12/96 is restated as follows: The Department of Veterans Affairs Medical Center, San Francisco, CA, intends to award a contract for the procurement of an indefinite quantity of home hemodialysis supplies for a base year and up to two option years. Bid opening date is changed to December 18, 1996. This is a combined synopsis for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented by dditional information included in this notice. This announcement constitutes the only solicitation for the following items; bids are hereby requested. A written invitation will not be issued by the Government. Solicitation No. IFB-662-02-97 is being issued as an Invitation for Bids (IFB). Home hemodialysis supplies are required by the Government pursuant to this solicitation/contract; the supplies shall comply with the following descriptions which include catalogue numbers assigned by the current contractor (National Medical Care (NMC)) for clarification. Dialysis equipment currently in use is Cobe Centry 2 RX; supplies shall be compatible with this equipment as appropriate. The items below are identified with a catalogue description; such identification is intended to be descriptive, but not restrictive, and is to indicate the quality and characteristics of products that will be satisfactory. Bids offering ''equal'' products product (including products of the brand name manufacturer other than the one described by number) will be considered for award if such products are clearly identified in the bids and are determined by the Government to meet fully salient characteristics of the item bearing the NMC catalog number. All items shall be priced for three years (Base Year (one year from date of award), Option Year I, and Option Year II); bidders not submitting prices for all items for all years shall be nonresponsive and will not be considered for award.
