DAAD01) A mini-proposal submission is required in response to this
notice. The purpose of this modification to the original notice which
was published on 10/17/96 is to advise prospective offerors that the
acquisition will be solicited using multi-phase techiques. This
modification represents phase one of this multi-phase solicitation.
During this phase the Government will use evaluation criteria to
down-select to those offerors who will be requested to participate in
subsequent phases. Failure to participatein this first phase by
submission of information responding to the evaluation criteria will
make prospective offerors ineligible to participate in subsequent
phases. The following evaluation factors will be used to evaluate
submissions. Weight designation for each factor is indicated in ()
after the factor. A red rating indicates failure to meet the minimum
requirements. A blue rating indicates acceptability. A green rting
indicates that the minimum requirements are significantly exceeded.
Factor 1. Contractor Experience (5). Contractor must have a minimum of
five years experience in same or similar services to receive a blue
rating and ten years or more to receive a gren rating. Factor 2. EPA
violations (5). Contractors with no more than three federal, state or
local violations in the past five years will receive a blue rating. No
violations in the past year will receive a green rating. Factor 3. Key
Personnel (4). Three core on-site personnel are required as follows: A
Program Manager, an Administrative Assistant and an Environmental
Engineer. These are consideredkey personnel. To receive a blue rating
the proposed key personnel must have a minimum of five years experience
in their field of expertise and ten years or more to receive a green
rating. Factor 4. Quality Assurance Plan (4). In order to receive a
blue raing the plan must be well organized and logical. In order to
receive a green rating the plan must demonstrate creative and
innovative quality assurance procedures that will result in excellent
performance. Factor 5. Training Program (3). An available training
program that ensures qualified personnel at all times will receive a
blue rating. In order to receive a green rating the program must
demonstrate anaggressive training program that includes creative and
innovative techniques. Factor 6. Subcontracting (2). Any proposed
subcontractors must meet all federal and state certification
requirements and have at least five years experience to receive a blue
ratng and 10 years experience to receive a green rating. Any
prospective offeror that meets the above eligibility requirments and
desires to participate in this acquisition must submit documentation
(original and one copy) as follows: 1) Synopsis of current and past
contracts that indicate company experience in performing same or
similar services. 2) Verifiable information regarding any EPA
violations. 3) esumes for key personnel. 4) Sample Quality Assurance
Plan. 5) Training Program documentation. 6) Information regarding any
proposed Subcontractors that evidences compliance with any
certification requirements and synopsis of current and past contracts
tha indicate experience in performing same or similar services. The
above information must be submitted no later than 5:00 PM MST, February
4, 1997. The Late Submissions, Modifications and Withdrawals or
Proposals Provision at Federal Acquisition Regulation 52.215-10 is
applicble to submissions in response to this notice. Submissions may
