Purchasing Agent (213) 742-2717. Transamerica Occidental Life Insurance
Company (TOLIC), which currently is under contract with HCFA for
Medicare Part B claims processing, is seeking sources for the printing
of Medicare Checks. Estimated volume for 12 months supply is
2,100,000. (I) TECHNICAL REQUIREMENTS -- Check stock must have the
following safety features: (1a) Basic Cascade Check protect -- Toner
Grip 25#, (b) brown stain, (c) void under coating, (d) tone grip
coating. (2a) Overall size 8-1/2" x 11", (b) size of check 8-1/2" x 3",
(c) size of Explanation of Medicare Benefits (EOMB) 8-1/3" x 8". (3)
Ink must be laser ink except for MICR encoding (black MICR ink). (4)
There will be three separate stocks on three different colors, screened
20% for custom Pyramid on upper portion. Colors -- black with warm red
background, Blue, and Green. (5) All pantographs must include the void
feature to protect against photocopy duplication. (6) Transamerica does
not use the standard ink colors for the void pantograph, vendor must
assumethe responsibility to print checks to ensure that the void
pantograph works correctly. (7) Disclaimer banner to be located on top
of check portion in Black. (8) Reverse side of check to show custom
step and repeat artificial water work Pyramids in fluorescent ink. (9)
Numbering must be printed in red ink in two locations -- numbering
must be continuous, without any breaks in event check number is missing
-- vendor must supply a list of checks and indicate missing numbers on
ream & box. (II) OTHER REQUIREMENTS: (1) Bank sheet to be provided for
MICR layout. (2) Monthly delivery of 350,000 checks to be shipped to
Los Angeles, CA 90054. (3) Cut sheet should be shrink wrapped in 500's
with clip board at top and bottom and packed 2,000 per carton in a
manner which will protect them from humidity and maintain their good
physical condition so they will feed and stack properly on Xerox 4135.
(4) Checks must be packed in reams with lowest #'s at the bottom and
the highest #'s at the top. (5) vendor must stock enough paper to
produce 500,000 checks for rush and/or disaster production. (6) vendor
must indicate other locations outside California capable of producing
check stock in the event of a disaster in California. All interested
vendors must submit their responses to include the following: (1a) unit
price, (1b) transportation & packing charge per carton, (1c) unit
charge for each safety feature, (2) 5 (five) client references, (3)
statement of previous experience, (4) SADBUS/MBE qualification
statement, (5) TOLIC onsite testing ability stmt. (6) qualification
statements stating the vendor's ability to meet the technical
specifications above (I- 1a-9), (7) responsiveness statement for
meeting the Other Requirements above (II 1-6) Vendors quotations will
be evaluated 70% on cost and 30% on providing and meeting the requested
information in technical and other requirements specified I & II above.

Purchasing agent at the address specifiedabove, by February 28, 1997.
This does not constitute a commitment by Transamerica Occidental Life
Insurance Company or Transamerica Insurance Corporation to acquire the
