Transamerica Occidental Life Insurance Company (TOLIC) which currently is under contract with HCFA for Medicare Part B claims processing is seeking for two high speed rotary transport Microfilm Cameras which feature operator programmability to allow the camera to be used in a variety of applications. (I) TECHNICAL REQUIREMENTS: Programmable features should include: one, two or three level image marking; batch processing; image management code (IMC) film marking. In addition, the camera should offer: Full keyboard and monitor; Patch code reader, Auto/manual feeder; Multiple/Interchangeable film units; Bar code scanner; Imprinter; Manual data entry; high speed film advance; Duo film capability; Film only function and varied document lengths and widths. (II) OTHER REQUIREMENTS: (1) The cost for 3 yr lease, purchase option, maintenance cost, key operators training, and equipment installation must be broken our separately, (2) five client references, (3) previous experience, and (4) SADBUS/MBE qualificationstatement. Vendors quotations will be evaluated 70% on cost and 30% on providing and meeting the requested information in technical and other requirements, in I and II above.
