W -- LEASE AND MAINTENANCE OF TWO HIGH SPEED READER PRINTERS POC Bob Collins Purchasing Dept., (213) 741-6270. Transamerica Occidental Life Insurance Company (TOLIC), which currently is under contract with HCFA for Medicare Part B claims processing, is seeking for two high speed blip sensing reader printers to produce minimum 500 high quality prints per day. (I) TECHNICAL REQUIREMENTS -- The equipment must include, at the minimum, the following: (a) ability to read 16mm negative dual sided tapes, (b) auto rotation, (c) 12" x 12" viewing screen, (d) print masking, (e) adjustable exposure and (f) high speed search. All machines must operate with a machine effectiveness rating of a minimum of 95% (II) OTHER REQUIREMENTS: (1) The cost for 3 yr. lease, purchase option, maintenance cost, key operators training, and equipment installation must be broken out separately. Maintenance service must be available Monday through Friday 8 a.m. to 8 p.m., (2) five client reference, (3) previous experience, (4) SADBUS/MBE qualifications statement. Vendors quotations will be evaluated 70% on cost and 30% on providing and meeting the requested information in technical and other requirements, in I and II above.
