C -- GREEN TEAM MODULAR BUILDING SOL RFP 508-54-97 DUE 030797 POC Jeff Miller Provide all labor, materials, tools and equipment, supervision and design-build services necessary for "Green Team Modular Building" at VA Medical Center, Atlanta, Georgia. Design-build as defined by VA is the procurement by the Government, under one contract, with one firm (which may be a joint venture) for both design and construction services. Work includes design, site preparation, and assemble a pre-manufactured modular structure arranged as required to form a primary health care facility to be located on the north end of the existing ambulatory care building and extend into the existing parking lot. The work will include site improvements, foundations, connections to utilities, fire protection, sidewalks, landscaping and paving. The structure will include finishes, heating, cooling, electrical, fire alarm, and intercom systems as required to a minimum of ten exam and ancillary services such as restrooms, waiting, offices, classroom, administrative areas, storage, etc. Approximate area of construction is 3000 square feet. The anticipated cost range for this project is between $250,000 & $500,000. All work to be in accordance with VA master specifications, VA design handbooks, Life Safety Codes, JCAHO Regulations, and all Federal, State, and local building codes. Contract duration: 180 days from NTP. Source should be a design-build firm that can demonstrate prior corporate experience in the last five years with design-build. A $50.00 deposit is required for all proposal materials. The $50.00 deposit must be submitted as a certified check, cashier's check, or money order. CASH, COMPANY CHECKS, & PERSONAL CHECKS WILL NOT BE ACCEPTED. If no proposal is submitted, please return proposal materials immediately. Within twenty (20) days after date that proposals are due, proposal material should be returned postage prepaid to the issuing office. At that time, proposal deposits will be returned to vendors who return COMPLETE proposal packages in GOOD condition. Vendors who return packages in either incomplete or in poor condition or both, or who do not return them within twenty (20) days after date that proposals are due will forfeit their $50.00 deposit.
