Scope of work is a follows; The contractor shall provide personal services of qualified dietition at the 16th Medical Group, Health and Wellness Center (HAWC), Hurlburt Field, Florida, as required by the Statement of Work (SOW). The dietition shall perform examinations and counseling to eligible beneficiaries of the military health care system who present themselves to the HAWC for assessment/counseling. Contractor dietitions shall be of quality, meeting or exceeding current recognized national standards as established by the Joint Commission on Accreditation of Health Care Organization (JCAHO) and the Commission of Dietetic Registration. Additionally, all proposed dietitions will make themsleves available for interview by the MTF during the credentialing process. All dietitions shall have a copy of valid, current membership in the American Dietetic Association, a copy of professional diploma, a copy of Cardiopulmonary Resuscitation Certificate, a certification of current physical examination, and proof of malpractice insurance, to include company and expiration date.
