The supplies which are required are 4C control Tri-pac, Uni-T-Pack, S-Cal calibrator, and patient form tickets. The delivery schedule is as follows: the 4C Control Tri-pac is required on a monthly basis and the remaining items are to be delivered on a quarterly schedule. The delivery period is 01 March 1997 -- 30 September 1997 with two one year options thereafter. All responsible small business' may submit an offer.
