The Indian Health Service requires Otolaryngology services, 12 months per year, one (1) full day of in-house surgery immediately followed by one (1) full day of in-house clinic per month during the term of this contract. Services to be performed for one year after award with four (4) one year option periods to extend for a total of five (5) years.
