The Stratton VA Medical Center, Albany, NY 12208, is seeking qualified firms who have ability to provide a health education tool to approximately 30,000 veterans four times a year. The government contemplates award of a firm fixed price contract for its base requirements of one yearly, with two annual renewal options. The successful offeror will be required to compose patient education material for specified cohorts from required data fields and will work with the VA to map appropriate data fields within existing VA database. Once data fields are mapped, the vendor will accept a file containing a delimited string of specified data fields and import such into vendor application to accomplish the health education information. Contract award will be determined on a combination of price, technical, and business-related factors. Contract requirements may include contractor performance of some onsite services at the Upstate New York VA Healthcare Network sites (Albany, Bath, Batavia, Buffalo, Canandaiqua andSyracuse).
