Boyd This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation is issued as a request for proposal (RFP). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 90-45. This is total small business set-aside. The associated standard industrial classification code is 5047 and small business size standard of 500 employees. Travis AFB requires an Infant Security System consisting of two (2) 4 ft. Infant -Pedi Security Systems; four (4) 8ft. Infant-Pedi Security Systems; three (3) Graphic Staff Alert Panels; one (1) Transmitter Tester; twenty-five (25) Alarming Band -- Receivers (covers 25 sq. ft.); six (6) Alarming Band Controller (4 Receivers); two (2) Alarming BandDeactivators (for discharging patients); three (3) Console Alarm Location, Date, Time, Transmitter, Printout; three (3) Roll Band Material; thirty (30) Infant Transmitters; six (6) DS37L-Scramble Pads; six (6) MB2- Mounting Box; six (6) MOV-35-Surge Protector; one (1) M8M-8 Door Access Control System (1,000 users only); one (1) SAM-1-Scramble Net Access Mfg. Software; six (6) UMK-ADA Universal Mounting Kit with Adjustable Slope; ten (10) Magnetic Locks; three (3) 9 amp, 15 volt; three (3) AIPhone-Video-Communication Entry System; and Fire Alarm connection, installed. FOB Destination Travis CA Delivery within 30days after receipt of order. The provision at 52.212-1, Instructions to Offerors-Commercial and the provision at 52.212-2, Evaluation -- Commercial Items and address the following evaluation criteria: Pricing Proposal. The offeror 's pricing proposal consisting of all cost or price information; A properly completed and signed Standard Form 129; Past Performance Information -- A list of contracts and subcontracts completed during the past three years and all contracts and subcontracts currently in process. Include the following information for each contract: Name of Contracting Activity; Contract Number; Contract Type; Total Contract Value; Contract Work; Contracting Officer and Telephone Number; Program Manager and Telephone Number; Each offeror will be evaluated on his/her performance under existing and prior contracts for similar products. Performance information will be used for both responsibility determinations and as an evaluation factor against which offeror's relative rankings will be compared to assure best value to the government. The government will focus on information that demonstrates quality of performance relative to the size and complexity of the procurement under consideration. References other than those identified by the offeror may be contacted by the Government with the information received used in the evaluation of the offeror's past performance. The offeror shall provide: A list of all proposed equipment which are provided with, and included in the unit price of, each system/configuration offered; Descriptive product literature for and performance specifications of each product, including all component items being proposed; The Government is not obligated to request any additional information from an offeror who does not provide sufficiently detailed information from which an accurate evaluation can be made of the offeror's proposal. Offerors shall include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications -- Commercial Items, with its offer. Clause at 52.212-4, Contract Terms and Conditions -- Commercial Items and clause at 52.212-5, Contract Terms and Conditions Required To Implement Statutes Or Executive Orders -- Commercial Items, applies to this acquisition.
