Rowland, 315-476-7461, ext 3599 This is a combined synopsis for commercial items prepared in accordance with the subparts in 12.6, as supplemented with additional information included in this notice. This notice constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 90-41. The Department of Veterans Affairs Medical Center, Syracuse intends to negotiate on a sole source basis with Connell Neurosurgical on behalf of Radionics, Inc. for a Cosman-Roberts-Wells Stereotatic System Compatible with CT. The system includes: Intubation Head Ring, Arc System, CT Localizer, Phantom Base, (2) preprogrammed computers, Mayfield headrest adapter, (4) head ring drives, (4) head ring posts, (4) head ring screws (long), (4) head ring screws (short), drill assembly 2.7mm, Guide Tube 2.7.1.D x 116mm, Reducing bushing 2.7mm, tap for cleaning head ring post threads, (2) head ring wrenches, Keller-Tiel Head ring positioner, Probe Microdrive and a pair of Retractor holders; on site installation and training. Written responses shall include prices and technical data sufficient to evaluate proposed system. The Government will award a firm fixed price contract, F.O.B. Destination. The Standard Industrial Classification is 5047. Offerors shall provide a completed set of provisions with proposal found at 52.212-3, Offeror Representations and Certifications -- Commercial items. Providsions and clauses found at 52.212-1, 52.212-4, 52.212-5 and 852.210-77 also apply.
