Thompson (90C) 615-327-5315 FAX 615-321-6351 The VA Medical Center, Nashville, TN is seeking offers from medical care providers for primary health care services for enrolled veterans. Payment for services will be on a monthly capitated rate basis. The contract term will be one base year with two 1 year options for contract extension. The primary health care services will be provided in two separate locations: one location will be in Christian County, KY and the second location will be in Warren County, KY. All facilities must be in compliance with the American Disabilities Act requirements. At the Christian County clinic facility, the VA Medical Center is expecting to enroll approximately 1400 patents one year after award. At the Warren County clinic facility, the VA Medical Center is expecting to enroll 1200 patents one year after award. The contractor will provide all personnel, clinic facilities, equipment, supplies, and supervision for the provision of primary health care services. The contractor will be responsible the provision of all radiological and laboratory services associated with primary health care covered by the contract. The contractor will be responsible for providing tract. The contractorshort term pharmacy prescription using the VA's formulary. The contractor must have a primary care provider on call 24 hours per day, 365 days a year to respond to enrolled patient telephone inquiries. Offerers may make offers for providing primary care at both locations or a single location. The VA will evaluate offers and make award either to a single contractor for both locations or two separate awards for each county location. The VAMC will make contract award to offeror or offerors that represent best value to the VAMC. Contract award or awards will be based upon the evaluation of offers using selection factors including cost as one selection factor. Solicitation documents are expected to be available on or about March 17, 1997. Offer due date is expected to be on about April 17, 1997.
