commercial items prepared in accordance with the format in FAR Subpart
12.6, as supplemented with additional information included in this
notice. This announcement constitutes the only solicitation; proposals
are being requested and a written solicitation will not be issued. The
solicitation # is 459-97-0008. This is a request for quotations, (RFQ).
The solicitation document and incorporated provisions and clauses are
those in effect through Federal Acquisition Circular 90-42. Items:

Offerors, please submit pricing for optional years as well. Items are
same as above, but for April 1, 1998 thru March 31, 1999 and April 1,
1999 thru March 31, 2000 respectively. REQUIREMENTS: 1. Contractor is
to provide all labor and materials to install one 1150 gallon tank for
liquid propane gas onto a Government provided concrete pad at the VA
Center for the aging located on the grounds of Tripler Army medical
Center, (TAMC). Installation and hook up must be in accordance with all
local codes and regulations (NFPA 58). Equipment utilizing LPG (liquid
propane gas) is, one oven range, 220,000 total BTU, and two water
heaters, 400,000 BTU each. Tank installation will include tank
supports, final piping connections, labeling and regulator required.
Tank supports must be anchored to pad and tank. Contractor will certify
by letter or other official documentation that their tank has been
tested and meets all local codes, regulations, and is ready for usage.
Contractor will ensure that the tank and fittings are free from
leakage at all times. Contractor will ensure the tank is rust free,
painted, labeled as required, and maintains a presentable appearance at
all times. Contractor will ensure tank filling/servicing is provided on
a regular basis so that the capacity is never below one third. Receipt
for the amount of LPG filled should be sent to Chief, A&MMS (90C), 300
Ala Moana Blvd, Room 3119A, Honolulu, HI 96813. Contractor will ensure
all safety requirements/notifications are posted when servicing the
tank. Any safety related discrepancies or hazards noted by the
contractor when servicing should be corrected immediately and/or
reported to the VA for correction. Contractor will provide a 24 hour
emergency number. Emergency service shall be provided within two (2)
hours of notification. Provision 52.212-1 INSTRUCTIONS TO OFFERORS --
COMMERCIAL ITEMS, applies to this acquisition. Provision 52.212-2,
EVALUATION -- COMMERCIAL ITEMS is applicable. The evaluation criteria
is, (a) Locality, and ability to respond in emergencies (50%) and (b)
Price (50%). Offeror must submit a completed copy of provision
52.212-3, OFFEROR REPRESENTATIONS AND CERTIFICATIONS -- COMMERCIAL
ITEMS with it's quotation. Clause 52.212-4, CONTRACT TERMS AND
CONDITIONS is applicable to this acquisition. Clause 52.212-5, CONTRACT
TERMS AND CONDITIONS REQUIRED TO IMPLEMENT STATUTES OR EXECUTIVE ORDERS
-- COMMERCIAL ITEMS, applies to this acquisition. In addition, all
applicable clauses and provisions from the VA Acquisition Regulation
