terms? STATEMENT OF WORK FIBER OPTIC EMERGENT REPAIR SUPPORT NAWC AD
LAKEHURST, NEW JERSRY 1.0 SCOPE 1.1 The contractor is required to
provide labor and equipment to support fiber optic cable damage repair
located at NAWC AD Lakehurst, NJ. This work will include site
surveillance to access repair needs and coordination with base
communications officer to use on-site equipment when available to
reduce the overall repair cost. 1.2 This effort will require a rapid
response to minimize computer network down time. Contact will be
initiated by NAWC AD Communications Office with a response required
within 24 hours. 2.0 APPLICABLE DOCUMENTS 2.1 EIA/TIA 568A, Commercial

SIP-003-020, Splice Close Installation 3.0 REQUIREMENTS 3.1 General
Conditions. (a) The contractor shall receive and respond as required to
all Fiber Optic Repair notices from the Government site Point of
Contact (POC). For the purpose of restoration of customer communication
fiber optic cables from any excavation or other damage. Other contract
responsibilities include, but may not be limited to, modification,
moves, adds, and changes required by the Governement in connection with
providing temporary or permanent network restoration. (b) The
contractor will be called by the Government site Point of Contact
(POC). Response time for normal or scheduled requirements shall be
within 48 hours. The response time for an emergent need requirement
shall be within 24 hours. (c) The contractor shall report to building

Devins. 3.2 Administration (a) The contractor shall provide such office
services and supplies as may be required for proper administration of
this agreement. Normal service hours and working days shall be 7 am --
5 pm Monday through Friday. (b) Administration responsibilities
includes, but is not limited to: receipt, processing, recording,
dispatch, and closing out of dispatch notices. 3.3 Repair Facilities
(a) Thecontractor shall provide all tools and material required for the
safe performance of this agreement. The contractor=s personnel shall be
equipped with paging devices to facilitate communications when on-site.
(b) Facility repairs shall be performed utilizing the Afusion spice@
method and encapsulated in a water tight splice enclosure for all under
ground and manhole repairs. (c) Pumping water from manholes may be
required for proper performance of this agreement and is the
responsibility of the contractor. (d) All safety equipment required to
perform repairs is the responsibility of the contractor and shall be
used at all appropriate evolutions. 3.4 Charts and Records The
Government shall provide accurate charts and records as may be
required. The contractor shall not assume responsibility or liability
for the accuracy of such charts and records. Charts and records shall
remain the property of the Government and shall not be removed from
Government property. 3.5 Contingency (a) Neither contractor nor
Government shall be held responsible for any delay or failure in
performance of a purchase order placed hereunder by Government caused
fires, strikes, embargoes, or other requirement imposed by government
regulation, civil or military authorities, acts of God or by public
enemy, inability to secure raw material or transportation facilities,
acts or missions of carriers or other causes beyond the control of the
contractor or customer. If such contingency occurs, the party injured
by the other=s inability to perform may elect to: 1. Terminate such
purchase order or part thereof as to Services not already received. 2.
Suspend such purchase order for the duration of the delay cause; or 3.
Resume performance under such purchase order once the delaying cause
ceases with an option in the injured party to extend the endured. (b)
Unless written notice is given within (30) days after such injured
party is apprized of the contingency, option A2@ shall be deemed
selected. 3.6 Right of Access Both contractor and Government
shallnormally permit access to their respective facilities by the other
in connection with work hereunder. No charge shall be made for such
visits. It is agreed that prior notification will be given when access
is required. Access will be provided during normal working hours. 3.7
Plant and Work Rules The contractor=s employees and agents and those
of the Government shall, while on the premises of the other, comply
with all rules and regulations. 3.8 Releases Void Neither contractor or
the Government shall require waivers or releases of any personal rights
from representatives= of the other in connection with official visits
to their respective premises and no such releases or waivers shall be
pleased by them or third persons in any action or proceeding. 3.9
Safety Requirements The contractor performing work under this contract
shall fully comply with the provisions of the Federal Occupational
Safety and Health Act (O.S.H.A.) and any rules and regulations pursuant
to the Act. 4.0 INSTRUCTION FOR BILLING SERVICES 4.1 Invoices shall
detail labor time, equipment rental expense, travel/per diem costs, and
installed equipment purchased itemized and shall not exceed the total
cost of this purchase order. See para. 7.4 for address to submit
invoices. 5.0 REQUIREMENTS 5.1 The Government POC shall contact the
contractor that emergent cable repair is required and the contractor
shall respond within 24 hours to provide a representative on-site to
assess the damages and provide an engineering plan and cost estimate
for repairs. 5.2 Preparation for on-site assessment, the contractor
shall bring equipment for normal outside fiber optic/copper cable plant
repair and after approval of the repair estimate, the contractor shall
immediately begin the repair evolution. 5.3 If heavy equipment is
required, the Government area POC and cognizant NISE East project
engineer shall be contacted to negotiate these services with local or
Base resources. 5.4 Equipment to have on-site during a call-out shall
include, but not limited to a fusion splicer, two direct burial splice
case enclosures with encapsulant and splice trays, fiber optic tool
kits and test equipment. Mechanical splicing devices shall also be
on-hand to restore temporary services until splices can be made. A
length of OSP burial cable suitable to patch a break in the existing
plant. 5.5 Existing excess cable from initial installation can and
should be used if available to minimize cost for repair and insure
proper cable match-up. An inventory can be made to assess the
availability of excess cable and equipment. Action should be taken to
set aside a sufficient amount of excess material for emergent repair.
5.6 Other repair work on a non-emergent call-out can be initiated to
repair various communication cable segments as required. Any such
call-outs shall have the cognizant NISE East project engineer=s
approval to ensure the scope of work fits the scope of work for this
purchase order and that sufficient funds exist to cover said services.
6.0 SEVERABILITY 6.1 If any of the provisions ofthis agreement shall
be invalid or unenforceable, such invalidity or unenforceability shall
not be invalidated or render unenforceable, the entire agreement, but
rather the entire agreement shall be construed as if not containing
the particular invalid or unenforceable provision or provisions, and
the rights and obligations of the contractor and the Government shall
be construed and enforces accordingly. 7.0 DELIVERABLES 7.1 The
contractor agrees to provide fiber optic facility repair Services as
outlined in the statement of work providing a 24 hour maximum response
time for emergent cable repair call-outs. 7.2 The contractor agrees to
furnish the Government with repair summary reports of repair progress
and cost during an emergent repair effort. 7.3 This purchase order
shall provide a minimum of 3 cable plant repair call-outs for normal
cable plant cuts. 7.4 Repair summaries shall be mailed or faxed to the
Cognizant Project Engineer, Will Rivers, NISE East Charleston, Code

974-4918. 7.5 Invoices shall be submitted to NISE East Charleston, Code
123, P.O. Box 190022., North Charleston, SC 29419 7.6 The period of
performance is from date of award through 30 September 1997. A SOLE
