LINEAR ACCELERATOR 2100C (1 EA), LINEAR ACCELERATOR (1 EA) AND
SIMULATOR LOCATED THE 60TH MEDICAL GROUP, TRAVIS AFB, CA SOL

procurement of commercial services with Varian Associates, Inc. for
preventative maintenance of the radiation therapy systems located in
the Department of Nuclear Medicine, David Grant Medical Center, Travis
AFB, CA. Maintenance of equipment includes Linear Accelerator models
2100c and 6/100, and the Ximatron Simulator with intervening repair
service calls including all parts and labor. Contract period will be 01
Apr 97 through 30 Sep 97 with provisions for four 12 month option
periods. This is a combined synopsis/solicitation for commercial items
prepared in accordance with the format in FAR Subpart 12.6, as
supplemented with additional information included in this notice. This
announcement constitutes the only solicitation; proposals are being
requested and a written solicitation will not be issued. The
solicitation number is F04626-97-R0002 which is issued as a request for
proposal (RFP). This solicitation document and incorporated provisions
and clauses are those in effect through Federal Acquisition Circular
92-15. This is not a set-aside, the associated standard industrial
classification code is 7699 and the small business size standard is $5
million. Services, Nonpersonal: To furnish all labor, tools, parts,
materials, facility and transportation necessary to provide quarterly
preventive maintenance and any/all intervening repair services on the
Varian Clinac 2100c Linear Accelerator, 6/100 Linear Accelerator, and
Simulator. The Contractor shall provide all equipment, tools,
materials, supervision, and other items and services necessary to
perform preventive maintenance and intervening service repairs on the
Linear Accelerator 2100c; Index 13688; Serial #022, Linear Accelerator
6/100; Index 10713; Serial #310, and Simulator; Index 10719; Serial
#032. Proposals shall include unit and extended prices for Customer
Support Plan II or equivalent. The Basic Performance period is from 1
May 97 through 30 Sep 97 with four 12 month option periods beginning 1
Oct 97 through 30 Sep 2001. Place of performance and FOB point is
David Grant Medical Center, Travis AFB, CA. The provision at 52.212-1,
Instructions to Offerors -- Commercial, applies to this acquisition.
The provision at 52.212-2, Evaluation -- Commercial Items, applies to
this acquisition, evaluation will be made upon technical capability of
the services offered to meet the Government requirement, price, and
past performance. All evaluation factors other than price, when
combined, are significantly more important than price. Offerors shall
include a completed copy of the provision at 52.212-3, Offeror
Representations and Certifications -- Commercial Items, with its offer.
The clause at 52.212-4, Contract Terms and Conditions -- Commercial
Items, applies to this acquisition. The clause at 52.212-5, Contract
Terms and Conditions Required To Implement Statutes Or Executive Orders
-- Commercial Items, applies to this acquisition The following FAR
clauses cited within that provision are also applicable: Paragraph (b)
(1), (2), (3), (4), (6), (7), (8), (9) and (c)(1), (2), (3), (5). The
provisions of FAR 52.217-8, Option to Extend Services, and FAR
52.217-9, Option to Extend the Term of the Contract (a) within 15, (c)
not to exceed 5 years, apply to this acquisition. The provisions at
DFAR 252.212-7001, Contract Terms and Conditions Required to Implement
Statute or Executive Orders Applicable to Defense Acquisition of
Commercial Items, applies to this acquisition. The following DFAR

Sea. Standard commercial warranty applies to the service purchased
under this solicitation and CBD note #22 applies. Vendors wishing to
respond to this solicitation should provide this office with the

showing unit price, extended price, prompt payment terms and correct
remittance address; and a completed copy of FAR Clause 52.212-3, Offer
