N -- X-RAY EQUIPMENT RELOCATION AT LUKE AFB, AZ SOL DACA87-97-R-0017 DUE 042897 POC Contracting Officer, Cpt Jose Baez, 205-895-1144, Contract Specialist, Chun Tucker, 205-895-1145 (Site Code DACA87) The US Army Engineering & Support Center, Huntsville intends to award a sole source contract to Philips Medical System, 4001 East Broadway #20, Phoenix, AZ 85040 for Deinstallation, Storage, and Reinstallation of the three Philips X-Ray units. The three Philips X-Ray machines are proprietary property of Philips Medical Systems. The performance location is 56th Medical Group, 7219 North Linchfield Road, Luke AFB, Arizona 85309. The contractor is responsible for Deinstalling three (3) Philips X-Ray systems (one rad/tomo system and two rad/fluoro systems) at Luke AFB Hospital, Phoenix, AZ. The contractor is also responsible for storing the equipment in a contractor-provided facility suitable for the purpose pending availability of the newly allocated hospital space. The period of storage will not exceed one year. Upon completion of the new facility, the contractor shall reinstall the three systems to the newly designated areas. In addition, the contractor shall provide and install a set of X-Ray railsfor each of the three systems (a total of 3 sets). All work shall be performed in accordance with manufacturer recommended procedures, and shall maintain the warranty and/or maintenance related services in accordance with the manufacturers specification. Upon installation, the contractor is responsible for ensuring that the system is fully operational. All work must be performed by personnel who are fully qualified and trained in the removal and reinstallation of this type of equipment. This is not a formal solicitation. The synopsis of intent to award a sole source contract is not a request for proposals/offers. Contractors who can provide the U.S. Army Engineering & Support Center with the above services are invited to submit in writing an affirmative response which includes, as a minimum, any applicable pricing data and any additional information indicating a bona fide ability to meet this specific requirement. In addition, contractors must provide documentation from Philips Medical System, which indicates that you are an authorized dealer or a representative in performing such services. All responsible sources may respond to this synopsis, and all such responses will be considered by the Agency. If no written responses are received within 45 calendar days, this acquisition will proceed on a sole source basis.
