Great Lakes Healthcare System Acquisition Center intends to negotiate
a sole source contract with McGaw Inc. for an Extended Perpetual
Maintenance Program (PMP) on the previously purchased Model 610 Horizon
NXT infusion pumps for a one year period with four one year option
renewal periods. -- SOL # 69D-205-97. This is a combined synopsis/
solicitation for commercial items prepared in accordance with the
format in Far Subpart 12.6 as supplemented with additional information
included in this notice. This announcement constitutes the only
solicitation; Request for Quotes (RFQ) are being requested and a
written solicitation will not be issued. Solicitation documents and
incorporated provisions and clauses are those in effect through Federal
Acquisition Circular 39. This procurement is UNRESTRICTED. SIC (CODE)
7699. Contractor shall provide to the VA Medical Center (Westside), 820
South Damen Avenue, Chicago, IL 60612 an extended perpetual maintenance
program (PMP) to include the training of one qualified technician to do
initial repairs of pumps. All necessary parts plus two instrument
service kits to initiate the PMP. Under this program the hospital
returns the defective component to the contractor on a freight-prepaid
basis. The contractor then returns a replacement component on a
no-charge, freight prepaid basis. If the defective subassembly is not
received by the contractor within thirty days, the VA will be billed
for the replacement assembly. The contractor reserves the right to
assess subassembly abuse. If subassembly malfunction is due to customer
abuse, nominal service charges will be assessed for repair as a result
of the abuse. In the event the VA is unable to repair the device, the
VA may return the device to the contractor. The VA will be charged for
only the labor incurred in repairing the device. Offerors are requested
to provide their most current catalogue price list, and the monthly
service costs for services. This will be a firm fixed price contract.
FOBis Destination. Payment will be made in arrears on certified
invoices. There are 175 pumps located throughout the Westside Facility.
