FAR Clauses 52.212-1, 212-3, 212-4, and 212-5 apply. Sealed bids will be accepted by this medical center for the award of a contract for prescription eyeglass services including lenses and frames on an as-needed (indefitite delivery/indefinite quantity) for veteran beneficiaries for the Dept. Veteran Affairs Medical Centers in Leavenworth, Topeka, and Wichita, Kansas; and Columbia, Kansas City, Poplar Bluff, and St. Louis, Missouri; and Marion, Illinois. Lenses shall be fabricated and provided in accordance with DFA Regulation 21 CFR Parts 3 and 4, for impact resistance. Frames shall be in accordance with "Americal National Standard Requirements for Dress Ophthalmic Frames. Delivery must be in 7 calendar days or less. Period of performance will be for one base year with four additional one-year options to renew. Contractor must be able to accept the Government purchase card for payment. This procurement is intended to be set aside for Small Business based on SIC 3851 a small business qualifies as small if it has less than 500 employees.
