This requirement is for a six month extension to the existing Contract No. MDA906-95-D-0008 for the Northern Six Region beginning May 1, 1997 through October 31, 1997. It is the Government's intent to modify the current contract to the incumbent contractor to perform a sole source extension unless an acceptable proposal is received from an alternative offeror. This extension includes the states of Wisconsin, West Virginia, Kentucky (with the exception of the Ft. Campbell catchment area), and Ohio. This firm fixed price contract will expire April 30, 1997. All responsible sources may submit a proposal which shall be considered by the agency. All offerors will be required to develop region wide networks of health care providers.
