criteria is as follows: A. Demonstrated Capacity (1) Physician
Experience/Personnel Qualifications: Number of years in practice in
Internal Medicine. Training and experience of physicians (specify by
name). Physicians providing services under this contract must be Board
Certified. Offeror's documentation shall contain a matrix that
identifies, physician name, board certification in Internal Medicine,
years experience in Internal Medicine, location of physician's resident
and citizenship. (Location of residence & citizenship are not
evaluation factors, but included for C&P purposes) See following
example: B/Cert (yes/no) Experience (Years and Months) Residence
(Location) Citizenship (U.S. or Other) B. Quality Assurance: Offeror
must provide as a minimum documentation of a quality assurance program
that will assure availability, response time and scheduling of
qualified staff, inclusive of backup staff and utilization of quality
of care monitors which measure patient outcomes and satisfaction (i.e.
mortality, complications & complaints). C. Price: Offeror will submit
a price for physician services. D. Past Performance/Prior Experience:
(1) Offeror must provide at least three references for verification of
past performance, inclusive of point of contact information and current
phone numbers. These references may include work at this facility. (2)
Company Experience: Offeror must provide number of years in business
providing the type of services required by the contract, in order to
meet the minimum requirement, a business must have 1 to 5 years
experience. Factor A & B are of equal value. Factor C is of less value
than Factor A or B. Factor D is of less value than factor C, however
subfactor 1 & 2 are of equal value. Offeror shall submit a completed
copy of provision 52.212-3, OFFEROR REPRESENTATIONS AND CERTIFICATIONS
-- COMMERCIAL ITEMS with it's quotation. ADDENDUM to 52.212-3, OFFEROR
REPRESENTATIONS AND CERTIFICATIONS applies and the following clause is
applicable VA Acquisition Regulation (VAAR) 852.219-70, Veteran Owned
Business (Dec 1990). Clause 52.212-4, CONTRACT TERMS AND CONDITIONS is
applicable to this acquisition. ADDENDUM to 52.212-4, CONTRACT TERMS
AND CONDITIONS applies and the following clauses are applicable:

852.271-70 Nondiscrimination in services provided beneficiaries(APR
1984). Clause 52.212-5, CONTRACT TERMS AND CONDITIONS REQUIRED TO
IMPLEMENT STATUES OR EXECUTIVE ORDERS -- COMMERICAL ITEMS, is
applicable to this acquisition. The following clauses in (b) are also
applicable: 52.222-26 Equal Opportunity, 52.222-35 Affirmative Action
for Special Disabled and Vietnam Era Veterans, 52.222-36 Affirmative
Action for Handicapped Workers, 52.222-37 Employment Reports on Special
Disabled Veterans and Veterans of the Vietnam Era. The Government
intends to award a firm-fixed price contract to the offeror that is
determined most advantageous to the Government, price and other factors
considered. The technical documentation shall contain in sufficient
detail information in order to enable government personnel to make a
thorough and sound determination based on the evaluation factors listed
and demonstrate that the prospective contractor has a thorough
knowledge and understanding of the requirements. Statements which
paraphrase the specifications or technical criteria are inadequate to
demonstrate this. If any offeror fails to meet the minimum stated
technical requirements, that offeror will be disqualified for
