The Indian Health Service requires Ophthalmology services, 12 months per year, two (2) full days of in-house clinic per month, up to two (2) full days of in-house surgery per month, and a possible one-half (1/2) day of laser treatment per month during the term of this contract. Services to be performed for one year after award with four (4) one year option periods to extend for a total of five (5) years. Requests for copies of RFP 97 0910 will be available approximately 15 days from publication. Bid opening date is tentatively scheduled for May 5, 1997.
