B -- LABORATORY TESTING SERVICE SOL NHLBI-PS-SP-97-364 DUE 041897 POC Cecilia Morales The National Heart Lung and Blood Institute (NHLBI) intends to purchase Laboratory Testing Services for approximately (2,000) two thousand Hematologic and clinical Pathologic animal samples. The period of performance is April 1997 through December 31, 1997. SCOPE OF WORK: The contractor must provide qualified personnel, materials, equipment, facilities and services to perform the tests. SPECIFICATIONS: 1) Samples shall be picked up by the Contractor from the National Institutes of Health (NIH) 900 0 Rockville Pike, Bethesda Maryland by 11:00 a.m. daily, including Government Holidays. The samples must be transported to the Contractor's laboratory and tested on the same day. The NIH Clinical Pathologist on staff will review the Laboratory results. The pick up schedule may be revised by NIH on a weekly basis. 2) The test samples must be handled and transported safely. 3) All tests results must be provided to the NIH facility by facsimile transmission within twenty-four (24) hours period and o riginal copies of the test results must be mailed to the NIH facility within one week. The Contractor must be able to provide test results on an emergency basis (less than 24 hours) at the investigator's requests. The Contractor must: 1) have a qualified clinical pathologist and a medical technologist as a permanent staff.; 2) Be experienced in handling nonhuman primate samples and interpreting the results.; and 3) Have a strong quality control and quality assurance program. The Contractor must have a favorable past performance evaluation. The animal blood samples shall be tested on the following: Hematology tests includes: Complete Blood Count (CBC), White blood cell differential (including cellular morphology) and Reticulocyte Count. Clinical Chemistry tests includes: Albumin, Total Protein, Globulin, Sodium, Potassium, Chloride, Calcium, Phosphorus, Alanine Aminotransferase, Aspartate Aminotransferase, Lactate Dehydrogenase, Alkaline Phosphatase, Gamma Glutamyl Transpeptidase, Creatine Kinase, U ric Acid, Glucose, Urea Nitrogen, Creatinine, Cholesterol (total), Triglycerides, Bilirubin (total), C0/2 and A/G Ratio.This requirement is under the Standard Industrial Classification (SIC) code 8071, Size Standard $5.0M. Quotations will be due fifteen (15) calendar days from the publication date of this synopsis. The quotation shall include a technical proposal outlining how the quoter proposes to complete the scope of work, a commercial price list with applicable Government discounts, courier service c harges per hour, prompt payment discount terms, business size and references from a minimum of three (3) previous contractor's including contact name, address, and telephone. SELECTION CRITERIA: Quotations will be evaluated on conformance to the government's Scope of Work, price, quality, and a proven record of successful past performance. All responsible sources may submit a quotation, which if timely received shall be considered by the agency.
